Assist a Certified Diabetes Educator/Registered Dietician in the development and coordination of a Nutrition & Wellness program for Beacon Medical Groups and Beacon Community Health Enhancement. Managed the Registered Dietitian/ Certified Diabetes Educator’s (RD/CDE) scheduling of referrals for eight Beacon Medical Group Clinics and completed calls to patients that showed signs of pre-diabetes or presented with diabetes, per physician referral.

Provide case management and education to individuals that have been diagnosed as having diabetes, pre-diabetes, other health impairments, or a combination thereof. Using multiple intelligence education measures, I taught patients in a way that promoted understanding and retention relating to their diabetes and/or other health diagnoses.
