The Trust provides progressive integrated mental health and learning disability services to a population of around 890,000 people. Credition and Okehampton Older People Community Mental Health Team comprises six community psychiatric nurses, a clinical team leader and qualified staff at Memory strategy groups.

* Provide direct clinical care and supported professional activities; undertake 2 outpatients clinics per week; 1 memory clinic every 4 weekly. On average I see 5 patients in my out patients clinic including new referrals and follow ups. I also see about 4-5 new referrals in my memory clinic.

This psychiatric training scheme is regulated by the Irish Psychiatric Training Committee (IPTC). This is a statutory body which is recognised by the Postgraduate Medical and Dental Board (PGMDB) and the Medical Council. It also receives educational approval from the Royal College of Psychiatrists in London.

There is an extensive teaching programme relevant to psychiatry within this scheme. Psychotherapy is available to trainees in all the main psychotherapy modalities. A mentoring scheme is established so that trainees can meet regularly with the same consultant psychiatrist.

* Experienced specialist approaches for the management of refractory cases of bipolar affective

disorder, depression and anxiety disorders.

The BHB runs the two hospitals in Bermuda. King Edward VII Memorial Hospital, the general hospital, has 236 acute beds and 104 elderly long stay beds. St Brendan's Hospital, the psychiatric hospital, has 25 acute beds and 96 long-stay and rehabilitation beds. The latter is the only inpatient treatment centre for the mentally ill and disabled in Bermuda. The Mental Health Act is Bermuda is very similar to the MHA (1983) for England & Wales. GENERAL ADULT PSYCHIATRY

I have held a total of 12 posts covering nearly six years, in adult psychiatry, from senior house officer to consultant level, during my career. These posts have been held predominately in the UK, but I have also enriched my experience of the specialty by training in different cultures abroad, including India, Bermuda and Australia. Moreover, all of this training has been undertaken in accredited and first rate medical institutions.

My training has equipped me with skills and experience in delivering high quality, evidence based, psychiatric care. Specifically:

* Proficient in the assessment, diagnosis and formulation of all forms of mental illness relevant to general adult psychiatry. This has been achieved through direct assessment and through harvesting and using effectively information gleaned from a variety of other professionals and carers.

* The ability to diagnose and manage all mental illnesses in adults They will have an understanding of the relevance of physical illness and the social, cultural, ethical and legal context in which the patient presents.

* Experience and understanding of the multi-agency health and social care environment in the context of developing and delivering complex care packages in accordance with National service frameworks/contemporary guidance for mental health.

* Providing clinical leadership in the management of complex cases involving multiple agencies, in accordance with the national service frameworks/contemporary guidance for mental health.

* Acquired a solid foundation of theoretical knowledge of psychiatry. In particular I fully understand and am able to apply Diagnostic and Statistical Manual of Mental Disorders (DSM) and the International Classification of Diseases (ICD) in my routine clinical work.

* Extensive and intensive experience within all adult psychiatry inpatient settings, working variously in the both public and private healthcare systems in Ireland, Australia, Bermuda and India. This I possess a valuable insight into the cross-cultural aspects of adult psychiatry in many settings.

* Developed to a high degree my clinical accumen and leadership skills in this setting and have on many occasions led multidisciplinary team reviews focused towards planning immediate and after care.

* Adopting a holistic and participative approach to the patients’ care with the families, crisis team and the local community teams improved the utilisation of multi-disciplinary resources.

* Offering my personal interest and expertise of psychological treatment such as counseling, using principles of cognitive behavioural therapy improved the patient’s awareness about their own mental illness and enhanced their coping strategies. This has improved the general outcome of illnesses.

COMMUNITY PSYCHIATRY

* Gained wide experience in community psychiatry: my most notable posts in this regard have included: Associate Specialist General Adult Psychiatry in 2007 for four months with the Community Mental Health Team, Hampshire NHS Partnership Trust; Staff Grade Psychiatrist Crisis Resolution & Home Treatment Team & Pendine Community Mental Health Team in 2005 with Cardiff and Vale NHS Trust; a Registrar post in General Adult Psychiatry, Acute Care Community Psychiatry and Early Intervention in Psychosis and Liaison Services at Gosford Hospital, for 4 months in 2003 New South Wales, Australia; and a Medical Officer post in Adult Psychiatry based at the Community

* Led the acute care community psychiatric team.

* Skilled and experience in outreach work, risk management, balancing crisis management

with routine tasks and working with families and co-morbid substance misuse.

* Valuable experience in working with the Crisis Resolution & Home Treatment Team. In

this arena I have acquired excellent skills and knowledge in helping clients to resolve the

crisis whilst providing support at home and thus preventing hospital admission.

* Attended training with the Sainsbury Centre for Mental Health.

SUBSTANCE MISUSE

* Involved extensively in the practical assessment and management of patients with

alcohol and drug abuse and dependence.

* Skills in identifying and prioritising interventions for associated physical,

psychological and social dysfunction, including when and how to arrange appropriate

after care such as residential rehabilitation or day programs.

* Good experience in approaches to engage and treating patients early in the

course of alcohol dependency.

* Gained tremendous experience in the management of the acute medical and psychiatric

complications of non-injectable substances of abuse.
