Implementation of Interqual for assessing patient caseloads from 12 to over 40 cases/day in the Acute Care Setting. Assisting to optimize patient care outcomes by communicating with doctors, nurses, insurance companies to provide the most appropriate, cost effective care for each patient. Working remotely, independently and as a team for various hospitals in the State of Texas from Brownsville to Houston. Flexible hours working weekends, Holidays, and prn. NURSE CASEMANAGER IN THE ICU/TELEMETRY/MEDSURG 11+ YEARS 2006 TO PRESENT

Assist in coordinating care plan with doctors, patients, staff and insurances when applicable. Complete timely case study reviews and submit to insurances using Interqual. Complete timely assessment of postop patients, administer IV/IM/PO medication appropriately to adults/children when needed: postop care levels for critical patients working independently as well as team work . As well as care and treatment for patients with mild wound care, provide discharge instructions when necessary.

Night LVN for approximately 1:60 nurse/patient ratio. Provide g-tube and peg tube feeds, IM medications, nightly assessments and treatments to them as necessary. Problem solving independently and with management and staff.

Evaluate for admission, potential patients for Long Term Acute Care. Discuss with families and patients the process of admission and care at an LTAC level. Coordinate with doctors, nurses, families’ updates on admission status for admission.
