Responsibilities include but not limited to providing and coordinating a wide range of social services to chronically and terminally ill patients and their families throughout the Hillsborough County region for Life Path Hospice. Participating with Interdisciplinary Group twice weekly providing support to patients and families to assist them with coping with illness, discharge planning, disease progression, developing Plans of Care and

participating in Goals of Care discussions, facing death and dying issues and the ultimate loss of the Patient as requested by Nurses or Physician group. This role coordinates community resources for Patients and families with their bio-psychosocial concerns and provides services in a variety of settings including home care, acute care, inpatient and residential hospice care long term care after hours and respite care. The Patient with Dementia, Alzheimer’s and COPD present unique circumstances of care demand and were a large part of role. Families are often at a loss with their caregiving requirements as disease progression encompasses the person.

This position validated patient wishes regarding EOL care inclusive of Advance Directives discussions around Health Care Surrogacy, DNRO, POLST, Full Code and final arrangements. Specifically, being on the Hospital Services Team, Patients admitted in hospital for decline to abandonment; are provided support, guidance and reassurance throughout the hospitalization. Assisting with death and bereavement support through presence at extubations and support care following those events are some additional services this role supports. Also, referrals of new patients to LifePath Hospice are seen by the Social Worker providing Informational Visits regarding services and programs available to Patients and families with LifePath, and obtaining consents for association with Hospice. Relationship building with Hospital staff, case managers, Palliative Teams and creating a community of teamwork and collaboration to assure the professionalism and commitment to quality care was a key component of this position. Special pilot projects as requested by leadership.

Reason for Departure: Transition to Core Navigation Process shifting needs of Patient Access Department. This role was extremely rewarding.

Responsibilities were to facilitate fluid transition into the Rehabilitation environment and encourage participation and provide services throughout discharge. This role was contracted for 30 days and continued to 45 days. This facility was relieved of entire Social Services Dept within two weeks of my coming on board and I functioned as the Sole Social Worker and Discharge Manager for 30 days for this 120 bed facility. Developing care plans, leading Care Plan Meetings with IDT, resident and family members in preparation for discharge. Developing planning in coordination with multi-disciplinary teams members as resident needs demanded coordination of care.

Responsibilities included being a Personal Health Coordinator in Clinical Guidance to over 400 Members of Humana-At-Home’s Medicare Enrichment Program. Client/Members presented with chronic illness of all etiology and severity. Client/ Members aged from 35 to 105 and encapsulated the Southeastern Region primarily of the United States. The latter part of this tenure Client/ Members in the Midwestern regions were added. With a roster of

over 400 members (345-405) on any given day, Coordinators were accountable for metric demands daily to build rapport, create plans of care, introduce community resources and invite educational opportunities in rotation of every 60 then 90 days. There were many opportunities to support members and connect them to Pharmaceutical directives and financial resources as demand guided the call. This position encouraged Members to gain understanding about their personal healthcare management and keep them on a path to wellness that benefited their relationship with physicians and insurance programs alike. This is a letter of reference for J. Shannon Figley, Social Worker. I hired Shannon in the spring of 2018 as a Social Service Specialist (SSS) for the hospital team at Lifepath Hospice. The goal of the hospital team is to facilitate safe discharge planning for the hospitalized patients at multiple hospitals throughout Hillsborough County, FL.

As a SSS, Shannon effectively deals with many psycho-social issues related to the patient’s life-limiting illness and their family issues. Shannon exemplifies accountability, as she always goes above and beyond to be available to the patients and families, often staying after her shift, even coming in on her days off, if that was the only way she could meet with them. Her attendance and promptness is excellent.

She demonstrates a genuine caring attitude towards all patients. For example, a baby of recently emigrated parents was on hospice services. Through an interpreter, Shannon learned that the young family was in need of many of life’s basic necessities to care for the terminally ill infant. Shannon organized a mini-drive for donated baby items and was successful in providing many of the items needed. This was greatly appreciated by the family.

Lastly, Shannon is a team-player, being an integral part of the multidisciplinary care team and well-respected by her colleagues. She works closely with the RN case managers, offering advice and counseling in dealing with difficult cases. She is an abundant resource for all disciplines.
