I am an L.P.N. with an associates degree in nursing, I have been employed with the Wake Forest Baptist Medical Center for 20+ years over the last 10 years I was a Clinical Research Coordinator/Protocol Specialist. I have worked in the Department of Nephrology, Hematology & Oncology, and the Department of Internal Medicine. My clinical coordination has ranged from trials in Hormone and Estrogen replacement to Cancer Research Trials….. Additionally, 9 years of Case management for high profile private duty clients that required high levels of organization, accuracy, and assessment & analysis for medical professionals while also providing attentiveness to patient & family needs as a part of providing excellent care. I am currently providing the following skills to a geriatric patient that I have cared for over the last 9 years, while being employed with Wake Forest Baptist Medical Center.

EXPERIENCE & SKILLS

During my tenure coordinating both medical and patient care cases, I provided and acquired the following experience and skills,

1. Applying knowledge and skills in accordance with accepted standards of clinical practice to facilitate problem resolution and achieve individualized patient goals and outcomes.

2. Maintain contact with patients, physicians, clinical manager(s), other members of the healthcare team in a timely manner regarding patient/family needs and status changes. Participate in care coordination activities and discharge planning as appropriate.

3. Maintain the highest standards of professional conduct in relation to information that is confidential in nature. Sharing information only when the recipient’s right to access is clearly established and sharing of such information is dearly in the best interest of the patient.

ADDITIONAL SKILLS

Over my most recent 10 years, I have been responsible for data management activities/tasks for principal investigators and research nurses engaged in clinical trials within the Comprehensive Cancer Center (CCC), which include institutional, cooperative group and industry sponsored trials. This position performs a variety of specialized duties with regard to assigned protocols including extracting and editing data, tracking, coordinating the submission of data internally and externally to the assigned cooperative groups as required, and communicating with staff and faculty members. Essential Functions:

● Coordinate the entry of new and follow-up patients onto protocols by gathering patient information and entering data into the appropriate institutional and/or Cancer Center clinical research management databases and by submitting the appropriate paperwork to internal and/or external agencies.

● Extract patient information from medical records charts and other paperwork for submission to coordinating center, including grading of toxicities utilizing standardized protocol charts.

● If applicable, edit patient information which has been extracted by ancillary institutions. Provide timeliness reports, quality control feedback, corrections, and necessary training to ancillary institutions.

● Monitor the compliance with protocols by maintaining a detailed knowledge of each protocol administered.

● Communicate with others, including but not limited to Protocol Specialists, departments, physicians, nurses, and institutions, regarding the patients on the protocols.

● Maintain a summary of the status of each research protocol by ensuring that all required data is entered into the database in a timely fashion.

● Contact patients and/or family members regarding the patient's status according to the established time frame.

● Monitor the hospital admissions of patients on the protocols in order to gather and report data in a timely manner.

● Participate in and/or conduct audits with regards to the assigned protocols.

● Maintain an organized and accurate filing system.

● Attend relevant investigational and professional meetings and keep abreast of current developments in the field.

Maintained CPI (Continuous Process Improvement)

Additional duties- maintaining the CPI (Continuous Process Improvement)is a report that is received by the transplant centers with a list of follow-up forms that are due in a given trimester. The forms are not counted as submitted until all errors have been resolved and all applicable inserts have been submitted and are designated as “error free” Special Projects – Data back to centers is a strategy for data management of a web based application that allows the center to retrieve data previously submitted. At selected time points the CIBMTR requests submitted data over a 3-4 year time period to be reviewed and to make corrections/adjustments necessary to “clean” the data prior to being published.
