- My quality improvement project focused on reducing heart failure readmission rate at Salem hospital.

- I initially did a literature review, and then with the help of Dr. Rabin we developed a checklist for HF patient management that would help physicians, nurses, and case managers to deliver the best possible care based on previous studies of the same issue. This checklist included many aspects of HF treatment such as acute congestion phase treatment, recognition of subclinical congestion, and the critical phase of transition of care aiming to implement many strategies simultaneously. We were successful to reduce our hospital readmission rate significantly.
