EXPERIENCE

Lakeview Nursing Facility, Grand Island, NE. August 2016-March 2017. Employed as Social Services Director. Duties and responsibilities included administering social and mental assessments of residents to ensure proper care and overall well-being, established policies for social services department, facilitated monthly resident council meetings, facilitated a multidisciplinary group for weekly family care plans, completed monthly audits for QAPI (Quality performance improvement) meetings and reported results to management team, completed MDS (Minimum Data Set, federally mandated assessments for patients in a long-term health care facility), received and amended all residents grievance reports, helped new residents with initial paperwork, helped residents file for Medicaid benefits, completed discharge summaries and referrals for residents, attended daily stand-up meetings with multidisciplinary team, supervised employees and volunteers, scheduled residents medical appointments and followed all facility policies and federal health care regulations.
