4/17 – Current Atlanta/G.V. Montgomery VA Medical Center Decatur, GA/Jackson, MS

Facility Program Manager/Clinical Pharmacist (40 Hours per week) – Ensure the facility and staff follow the policies and guidelines of USP 797 and 800, as well as the standards of Joint Commission. Develop standard operating procedures (SOPs) for the facility. Lead a multi-disciplinary team that works together to ensure hospital wide compliance when handling hazardous drugs. Collaborate with other departments to develop processes to improve patient care. Worked as the backup to the Informatics pharmacist. Trained on Pyramid and SQL. Worked on the Pyxis ES build and to build the drug library for a new drug delivery pump. Enter orders using the Vista system and ensure that the prescribed drugs and doses are safe and appropriate. Contact providers to make renal dosing adjustments when necessary. Use sterile technique to compound chemotherapy and other iv drugs. Use the EMR to review patient’s medical records to prepare medication reconciliation booklets for patients and use the contents of those booklets to counsel patients upon discharge. MTM Pharmacist Consultant (40 Hours per week) - Provided evaluation of members’ Rx needs and therapies through review of medication history to determine if medications are appropriate based on diagnosis and concomitant co-morbid conditions. Assessed member compliance with plan through direct contact with members and their physicians. Determined potential barriers to compliance and identifies solutions to overcome barriers. Ensured that members on our Medicare plans understood appropriate use of prescriptions.
