Direct support staff – February 6, 2016 – november 2019

Overnight Direct Support Staff – (All men house), February 6, 2016 –July 20, 2016

As the overnight Direct Support Staff (40 hrs or more) I worked at Bermuda House and Chel wood House which were group homes for individuals with high level disabilities. My duties included checking on the individuals during the night in their rooms and repositioning and assisting them throughout the night. I assisted a non high functional individual walk to the restroom using hand over assistance with their walker walk to the restroom, assisted with taking vital signs, and assisted them with changing Depends and applying Desitin ointment throughout the night. I also assisted individuals with walking with a gate belt using hand over assistance. In the mornings my duties included preparing breakfast, packing lunches so the individuals can take to day hab, mopping, sweeping, taking out the trash, assisting with administering medications, writing notes in the overnight supported living progress note, filling out medication temperature log and recording in the medication administration record and recording in the breakfast percent-eaten log. I assisted individuals apply daily ointments and assited them with the use of a catheter in the restroom,and also completing the daily overnight cleaning log.

I also transported the individuals to day hab using the company’s vehicle (truck) and my own car. I tracked the miles on how long the trip was using the milage tracking log from the beginning odometer and ending odometer readings. I was reimbursed at the end of the month for all the miles I drove with individual in my car.

Direct Support Staff – Day shift, July 20, 2016 – February 2017

After working 7 months, I switched to day shift as the Direct Support Staff: at Los Camas, Santo Domingo, Douglas MacArthur Houses; Bryan House and Lewis Court House. I worked 12 hr shifts Monday through Friday, forty of more hrs a week. My duties included cooking, cleaning, assisting with hygiene, restroom assistance, medication delivery and administering meds, transporting and assisting individuals with meaningful day and community activities. I wrote notes thoughout the day on individuals’ in the Supported Living Progress notebook, and completed the individual’s outcome tracking. This included assisting individuals with their Health Outcome. I assisted them with following and establishing a visual hygiene routing and rating scale. For level of prompting, I assisted individuals with their live area outcome with learning to use kitchen appliances and utensils, and about proper nutrition and safe food handling practices. I also taught them about menu planning, budgeting, and grocery shopping, and using the rating scale for level of prompting. I assisted individuals with their Have Fun/relationship out come. I assisted individuals choose, plan and participate in activities of their choice from their Fun list, four times a month. I attended mandatory training, including: Positive Behavior Support; Strategies For the Department of Health. Assisting with medication delivery, Practice Comm DS/CM/ISC; Teaching and Support Strategies; Advocacy 101; Defensive Driving. I also worked overtime when asked by my supervisor or service coordinator, or another staff who could not show up for work.

Direct Support Staff – Weekend Day Shift: Sam Bratton House (Women only house)

I was promoted to work the weekend shift which was Friday through Sunday from 8am to 8pm. I worked 40 or more hours per week. My duties included cooking, hygiene, lifting, assisting with med delivery, transporting, cleaning, outcome tracking, mileage tracking, writing in supported living progress notes, and assisting the individuals with laundry and light housekeeping. I worked with individuals that were obese, deaf and in wheelchairs.

Family Living Home-based Support Provider: Lewis Court location

As a Family Living Support Provider I worked overnight during the week, 12 shifts or more, assisting the individual throughout the night by checking on them, repositioning them, and using hand over assistance, by changing Llestomy bags and emptying them. I also wrote in the overnight supported living progress notebook, cooked breakfast. With this position I was a private contractor and I was paid $110/day if I worked 13 hours or more; or $9.10/hr if I worked less than 13 hrs.

ALBUQUERQUE RESCUE MISSION (WINTER EMERGENCY SHELTER) SHELTER MONITOR VAN HELPER NOVEMBER 2015-MARCH 2016

As a shelter monitor I removed the log book, and van keys, and cell phone from the guest services help desk. I ensured that the vehicle had enough fuel for my shift. I then signed in homeless individuals that we picked up from designated stops throughout central and from the Albuquerque rescue mission down town, and signed them into the passenger log book. I also helped disabled homeless individuals onto the bus by using the bus lift for individuals in wheel chairs and walkers. From there we transported the individuals to the west side emergency winter shelter located at the old MBC JAIL. I also completed daily vehicle maintenance checklist forms, and emergency van checklist forms. I also made sure the emergency van was clean daily, by sweeping, mopping, and picking up trash off the floor.

Reason leaving: The season ended, it was a part time night job that was 36 hours a week from 6pm-12am

AMBERCARE (PERSONAL CARE ATTENDANT)

As a personal care attendant I performed task’s that contribute to the consumers daily living needs. I attended to the consumers requests promptly, and performed assistance with daily living activities that included Bowel and Blatter services, Bowl care-evacuation and ostomy care, Meal Preparation and assistance, prepared meals including cutting ingredients to be cooked, cooking of meals, and presenting meals in front of the consumer to eat. Eating, feeding the consumer or assisting the consumer with eating a prepared meal with a utensil or with specialized utensils. House hold support services, sweeping, mopping, vacuuming, changing the consumers linens, washing the consumers laundry, cleaning the consumers bathroom, kitchen and dining area. Assistance with battery replacement in Dual Medical equipment, and assistance with self-administering physician ordered prescriptions. At the direction of the consumer, handing the consumers his/ her daily medication box or bottle, and getting the consumer a glass of water or liquid for the purpose of taking medications. Hygiene and grooming, dressing, grooming, oral care, and bathing. Supportive mobility assistance, Ambulation- moving around inside or outside the residence of consumers living area, transferring from one location, and repositioning, turning or moving an individual to another position. I also attended required in service and programs per agency policy, and maintained good punctually and attendance record.

THE MONTEBELLO ON ACADEMY (FIVE STAR SENIOR LIVING FACILITY) LAUNDRY ATTENDANT MARCH 2015- MAY 2015

As the laundry attendant I had the responsibility of washing the seniors clothes, sheets, blankets, pillow cases, table cloths from the dining hall, towels, wash cloths hand towels, and face towels. I also pushed a carton throughout the facility and picked up dirty linen from the healthcare department and independent living department, and the shower room. I also picked up dirty table cloths from the bin in the kitchen, washed them and dried them. I dried clothes, towels, wash clothes, hand towels, and face towels. I also folded sheets, blankets, towels, face towel’s, wash cloths, put them in a basket and took them upstairs to the cabinets they belong to in health care and independent living. I had the responsibility of also folding table cloths and taking them upstairs and putting them in the IU crate next to the kitchen, I also sort them by color. I also logged in senior clothes, and the room number that house keeping staff brought to us, I also would sort them by color and add in soap so they could wash. I also sprayed the washing machines down every day so they could stay clean, and I would sweep and mop our department everyday, and also the break room, I also took out the trash from our department, and the break room everyday.
