Coordinate admissions with patient/family/physicians, obtain all required documentation. Communicate daily with doctors and other healthcare professionals, supervise all intake support staff, review all documentation on start of care oasis, case manage new patients until chart fully prepared, while doing all ICD-10 coding on all new starts. Previous to Intake Coordinator/Director I have worked as LPN in the field for over ten years performing various skilled nursing duties including detailed head to toe assessments, phlebotomy, wound care, catheters, teaching on diagnosis and medications, with attention to keeping patients safe in their home environment. I advanced my skills by becoming head of data entry putting all skilled nursing, therapy, physician orders into 60-day summary to be reviewed by ordering physician. I worked diligently with QA director/CQI chairperson to compile and implement training for all new and current employees. The last year I moved up to Assistant Intake Coordinator putting plan of care for all new admissions and readmissions.
