RN 2000 (Night Shift)-Present

• Care for post acute care patients with acute

and chronic complex medical conditions with

concomitant comorbidities.

• Care for patient’s with post-surgical orthopedic injuries, joint replacements, advanced surgical joint repairs & other major surgical procedures.

• Obtain patient health history, H & P and identify risk factors which contributed to hospitalization.

• Educate & counsel patients using preventative

strategies to reduce risk of re-hospitilization.

• Implement risk reduction interventions including life- style behavior modification to improve and maintain optimal health.

• Initiate & implement discharge plans on admission.

• Conduct admission & discharge medication

reconciliation.

• Assess health literacy and proficiency to improve self care.

• Collaborate with attending physicians &

interdisciplinary team members to create medically appropriate treatment plans to optimize patient

outcomes and health promotion.

• Monitor & interpret inflammatory markers, laboratory data, imaging, diagnostic test to monitor for

treatment efficacy.

• Episodic notes reflecting acute changes in condition.

• Occasional comprehensive progress notes on

admissions & overall significant changes.

• Telestat unit for cardiac health, Hospice & palliative care for end of life care.

disorders including viral hepatitis,

carcinomas.

• Observe Upper endoscopy & colonoscopy

in state of the art GI suite. Perform targeted

assessments for acute conditions.

• Comprehensive & target assessments for new

patients, follow ups & surgical clearance

• Diagnose & treat acute & chronic medical

conditions

• Order lab, imaging & diagnostic test to R/O

or confirm disease
