I am a nursing preceptor which allows me to train new nursing staff. During my shift I am responsible for making sure all my patients are safe, pass medications, provide wound care, assessing patients and notifying the physician and RN when needed with any change of condition. Being a field clinician, I would go to patients homes who had recently been discharged from the hospital. Patients would be assessed and if there was any change of condition the physician would be notified. I would have to also keep good communication with my assigned RN Case Manager and give them updates on the patient. Also performing wound care, making sure medications were updated weekly, and also teaching patients and/or family members on disease processes/wound care/medications.
