Work History
Skilled Nursing Administrator, 07/2018 to 01/2019
The Bradley Court – El Cajon, CA
Operated 56 bed 5-Star rated building with 28 secured beds
Led team through 3 surveys (Title 22, OBRA, and Life Safety) within two weeks of starting and had best results building had experienced in years, if not ever
Initiated weekly stand-up meeting that building was not utilizing
Took part in first county-wide Disaster Preparedness Drill
Kept census full and increased Medicare
Helped to lower psychotropics, decrease complaints, and minimize other compliance issues
Skilled Nursing Administrator, 12/2017 to 05/2018
Sky Harbor & Desert Manor Care Centers – Yucca Valley, CA
Contracted with Braswell's Medical Consultants, Inc. to manage/operate both Sky Harbor Care Center (99 beds) and Desert Manor Care Center (58 beds) in Yucca Valley, CA
Sky Harbor had census of 80 with 3 Medicare on December 4th and by the 10th it was 94 with 10 Medicare and we hit 27 Medicare in February
Accomplished both with census and revenue (increased operating revenue over $250K per month during my first 45-days) increases that owner acknowledged, "he had never experienced in over 50 years as an owner/operator in skilled nursing"
Brought back a number of staff that lived in the area but refused to work in these buildings due to bad reputation and history
Skilled Nursing Administrator, 01/2015 to 03/2017
California Nursing & Rehabilitation Center – Palm Springs, CA
Was hired by owner of company to "fix reputation of building" and "improve relations with Desert Oasis" who leased 30 of our 80 beds, of which was accomplished
Increased revenue and net income (according to 2015 OSHPD financial data reports) by $1.7 million and $500K respectively
Implemented much needed morale building programs such as B-Safe Bingo, Employee of the Month, and annual well-organized Christmas Parties
Brought previous DON (who spent 25 years with previous company) to come help me; of which, shows both loyalty and valued leadership
Day before officially taking over Desert Springs Healthcare (68 bed facility in Indio, CA), CDPH would have issued 2 IJ's had what they required, not been accomplished before leaving that day
Learned invaluable lessons splitting time between 2 buildings, of which one (CNRC-80 beds) was like running 3-4 buildings averaging 100+ admission/discharges/month
Before leaving company, negotiated new contract from $365/day to $475/day and not long after leaving, Desert Oasis pulled their contract and left the building. Recently learned that if I was willing to come back to CNRC, they would reinstate contract
Skilled Nursing Administrator, 04/2012 to 12/2014
Palms Springs Healthcare & Rehabilitation Center – Palm Springs, CA
Took over a financially challenged (-$300K) building in April of 2012 and had initial assessment been supported, would have turned that negative into a positive first 8mos
By establishing good relations with HMO and maintaining good case mix, building continues to produce positive revenue
Implemented morale building programs such as B-Safe Bingo, Employee of the Month, and annual well-organized Christmas Parties
Led team to produce three outstanding annual and Life Safety surveys; last of which, ONLY produced two minor deficiencies none towards quality of care
Effectively managed staffing, costs, case management, and morale, going from 30-40 admission/discharges a month to over 100/month
Established one of, if not the lowest, hospital readmission rates in the desert
Built a positive relationship with discharge planners, hospitalist's, and other administrative personnel at our area hospitals that resulted in increased Medicare
Was persistent in getting corporate to enclose facility for patient safety and other compliance issues
Skilled Nursing Administrator, 06/2010 to 01/2012

Worked with the Mayor of Barstow and several other organizations to bridge a gap between the home and the community
Implemented an employee of the month program and facilitated a Christmas party planning committee to raise money for event
Led team through successful surveys from USDVA and CDPH maintaining the homes 5-Star rating
