General Information: I am a licensed PTA in the state of Tennessee and have practiced in this state for 29 years. SNF’s are the setting that have the most experience in, but I also have practiced in Outpatient and Homecare settings. I have extensive continuing education in Geriatrics, Orthopedics, McKenzie and Myofascial Release techniques. I am an easy going person and a team player with supervisory, clerical and interpersonal skills who delivers quality physical therapy in an efficient manner.

Work History

08-2019 to present: Caring for parents and family. This included caring for my father after hospital stay due to CHF and mother after CVA.

2012-2019 NHC SNF’s and Homecare in Tennessee

During my years with this company I worked in several centers, homecare and treated patients in the outpatient setting. I utilized state of the art medical equipment and software to complete my duties assisting several staff Physical Therapist. I provided patient care, completed all documentation in a timely manner and was a clinical instructor for PTA students.

2005-2011 Signature Health Care of Fentress County, Jamestown, TN

At this facility, I worked as a PTA providing one on one and group therapy to patients while coordinating with rehabilitation team of occupational, physical, and speech therapies. The team experienced growth from 4 therapists to 11 in six years which allowed me to develop improved interpersonal skills. I trained local high school students as well as nursing/therapy students in local community college in therapy procedures completed in this setting. I also assisted with MDS completion and coordinated activities between Activities and Therapy departments.

1999-2005 Allied Medical Center/ Gateway Chiropractic Center, Crossville, TN

I treated patients in this outpatient clinic for a variety of spina issues such as scoliosis, bulging disc, and pinched nerves. Other disabilities treated here included shoulder dislocations/ rehabilitation after surgical procedures, total knee replacements, gait disturbances and balance problems. Modalities used included electrical stimulation, ultrasound, massage and traction.

1998-1999 Sundance Rehabilitation Corporation, Fentress County Rehabilitation Center, Jamestown, TN\

I worked at this SNF providing quality one on one and group therapy to geriatric residents utilizing state of the art modalities for direct patient care. I contributed to completing MDS and Care Plans for all residents of this 140 bed facility.

Work History

1997-1998 Functional Pathways of Tennessee, Byrdstown TN

While employed with this company, I worked one on one with geriatric residents utilizing modalities such as electrical stimulation, ultrasound, diathermy and exercise to complete their rehabilitation. I was responsible for completing MDS and Care Plans pertaining to therapy and entering data in the computer.

1990-1997 Professional Health Services (NHC) at Springfield, TN for 6 years and Floater position in middle TN for 1 year.

While working here I provided quality care in SNF’s, Home Health and industrial companies such as Frigidaire. After completing two years as a staff PTA, I was promoted to co-department manager supervising two technicians under direct supervision of PT. I then worked as a traveling therapist for this company in many facilities in Tennessee. I also participated in the Care Plan meetings and was chairperson of the safety committee for one year.

A.A.S. Volunteer State Community College in Gallatin, TN. Graduated in May of 1990.
