1992-1996 Salt Lake City, Utah

University of Utah Hospital, Director of Orthopedic Therapy. Acute Care-Orthopedics. Responsible for fifteen to twenty-five patients per day. Supervised all Physical Therapists in the department. Worked very closely with Orthopedic Surgeons.

1996-2000 Tooele County, Utah

Columbia/ Heritage Home Health Care, Director of Rehabilitation and primary physical therapist. In 1999-2000, my privately owned company named Pinnacle Physical Therapy, contracted all therapies to Heritage Home Health Care.

1999-2007 Tooele County, Utah

I owned, operated and directed – Pinnacle Physical Therapy Clinics. During this time I developed pool therapy, woman’s health, sports medicine with two high schools, house call therapy, inpatient and outpatient programs and home health contracts with multiple home health agencies. As Owner and Director, I was designated as Director of Rehabilitation of Mountain West Medical center (hospital). I was responsible for the management of over twenty staff and three outpatient clinics as well as assisting the hospital in attaining JCAHO Gold standing. Very well versed in regulations and Laws governing these settings. During my tenure at Pinnacle Physical Therapy I served on the Board of Director’s of the Tooele Chamber of Commerce from 2004 to 2006. Practice was sold in December, 2007.

•2008 Salt Lake City, Utah. Albuquerque, New Mexico. Westminster, Colorado

Navitus Cancer Rehabilitation. Director of Clinical Operations, I was responsible for decreasing expenses fifty percent and raised gross revenue forty percent. This was the highest productivity in the company’s five-year history. I developed and executed physical therapy programs for patients with oncology –related disabilities, participated in clinical research and education programs.

•2008-2009 Buffalo, NY

Supplemental Health Care. Served as a contracted Physical Therapist in Home Health and skilled nursing facilities. Evaluated patients with a wide variety of disorders including multiple medical problems, status post Joint Replacement, CVA's and many other injuries or disorders.

•2009-2010 Buffalo, NY

Served as Physical Therapist in Williamsville Manor skilled nursing facility. Responsible for Evaluations and treatment of many disorders in Multiple building locations. I enjoyed the variety of treating multiple medical disorders and injury recovery.

2009 – 2011

Provided Physical Therapy services for Pinnacle Physical Therapy Solutions, PC. This is a personally wholly owned and operated business that contracted skilled services to facilities in need of Physical Therapy services. We provided skilled PT to skilled nursing facilities, home health care agencies, and an adult developmental center that provided Physical Therapy. In addition to myself the company employed one additional Physical Therapist, a Physical Therapist assistant and office manager. The company was successful immediately making a gross income of over $182,000.00 in its first year.

2012 – 2013 Tooele, Utah

Provided skilled Physical Therapy to Rocky Mountain Care, LLC. Provided skilled services on inpatients of a skilled nursing facility, home health care, and outpatient therapies. These three settings were often done on the same day. Responsible for building an outpatient practice in a skilled nursing facility with a growth rate of 1 to 10 patients within 3 months. Left the company due to getting married and needing to move to Jacksonville Florida. Primary purpose of move was having a new family of 5, a new baby, and financial reasons.

2014 – Jacksonville Florida – Coastal Physical Therapy

Provided skilled Physical Therapy to patients in an outpatient Therapy Facility. Within 4 months I became Clinical Director. Within nine months I brought the clinic to profitability with a very high patient satisfaction rate with average of 20 patients treated per day. Left company due to questionably ethical practices suggested by management. This was a Physician owned company.

2014 – Panama City Florida – Advanced Medical Contract Physical Therapist

Provided Skilled Physical Therapy for patients at a Physician Pain Management Clinic. I worked closely with and supervised a skilled PTA. We treated approximately Fifteen patients per day. This was a six month contract of employment. I was able to get the Skilled Physical Therapy part of the clinic out of a $30,000.00 debt and into profitability in the six months working there.

2014-2016 – Panama City Florida – The NeuroMedical Institute

I started and grew a dormant Skilled Physical department by providing management, development and clinical services to referred patients. Within 7 months I was able to increase the clinic to over ten patients treated per day and to profitability. We hired a skilled PTA, who was a new graduate from school. I supervised and mentored our skilled PTA who developed exceptional clinical skills. After working there for 16

months the therapy patient caseload grew to the point of needing another Physical Therapist. I also participated in State of the Art Neurological Research. Left the company due to my growing family needing to move to our house in Jacksonville Florida for my growing family.

2016-2018 Home First Home Health Care

I worked for Home First Home Healthcare providing skilled PT services in patient's homes. I became well versed in the latest CMS rules and regulations. I did SOC's, treatment, Discharges and supervisories for HHA's and PTA's. Left for better employment.

2018-2019 North Florida Medical Center

I worked in an outpatient setting covering four locations and providing skilled PT services. I was promoted to Director of Rehabilitation after 3 months of employment. I supervised PTA's, PT Aides, and Massage Therapists. I left the company in July 2019 for personal reasons.
