Advance Auto Parts (“the Company”), as well as any of its affiliates, subsidiaries, agents, parents, partners, joint venture partners, and assigns (hereinafter “Advance Auto Parts”) may obtain information about you for employment purposes from a third party consumer reporting agency. This report is sometimes referred to as a “consumer report” and may include information about your character, general reputation, personal characteristics, and/or mode of living. These reports may also contain information regarding your credit history, criminal history, social security verification, and motor vehicle records

(“driving records”). Credit history will only be requested where such information is related to the duties and responsibilities of the position for which you are applying.

AUTHORIZATION

I hereby consent to this investigation and authorize the Company to request a report on my background as stated above from the consumer reporting agency at any time after receipt of this authorization and throughout my employment, if applicable.
