HOMELESS OUTREACH CASE MANAGER CASA INC./ DMHAS/HOT TEAM MARCH 2023-PRESENT

Homeless Outreach Team (HOT), working with homeless clients, persons with co-occurring disorders, HIV, and other comorbid disorders.

Facilitate connecting persons with mental health, housing, clinical, and substance abuse services—advocate for homeless individuals in the Bridgeport area and the surrounding towns.

Provide outreach services to clients who frequent bus stations, train stations, parks, soup kitchens, and other locations. Assist homeless clients with housing applications, timelines, and pertinent document gathering.

Acts as a liaison between section 8, housing managers, ABH, and other service agencies, Efficient on Rapid Exits, HMIS, WITS, VI-SPADAT, and client transportation services. Multicultural competent.
