Dear Deborah,

On behalf of Help at Home, I am pleased to extend a Conditional offer to you for the

position of Home Care Aide in our Alton, IL 62002 location. In this role, you will report to the Branch Manager. The terms of our Conditional offer are as follows:

• Hire Date: Your anticipated start date is August 08, 2024.

• Compensation: Your base compensation will be $17.00 / per hour. Upon hire, you will be paid weekly, and your earnings are subject to usual taxes and withholdings.

• At-Will Employment: The employment relationship at Help at Home is “at-will," which entitles you or the company to terminate the employment relationship at any time for no reason or any reason, without providing prior notice, warning, or discipline. Additionally, the terms and conditions of employment, including compensation, benefits, and privileges, can be changed, or terminated without cause and without notice at any time, at the sole discretion of Help at Home. Employees covered by a Collective Bargaining Agreement will be guided by the provisions contained therein.

• Conditional Offer Contingencies: This offer and your employment is contingent upon the below provisions. Please note that your failure to successfully meet these expectations will result in this conditional offer being rescinded.

- You must provide documentation confirming your legal right to work in the United States, according to the Immigration and Nationality Act (“INA”), within seventy-two (72) hours of your first date of employment.

- You are required to successfully complete a background check that will include a criminal history search.

- Your completion of 24 hours of initial pre-service training. This unpaid training is mandated by the State of Illinois and is offered free of charge. Help at Home, Inc. does not guarantee work to any person who has completed pre-service training. Pre-service training is a requirement of our contracting sources and is required for employment eligibility. Completing pre-service training neither guarantees you will be given a client to provide services to nor does it guarantee any number of hours. The completion of the pre-service training and all required documents only means you are now eligible to provide services to a Help at Home, Inc. client.

If the above terms are acceptable, please sign and return a copy of this letter within three days of the date of this Conditional offer.
