1 Florida Health Connect Product

1.1 What is Florida Health Connect?

The Florida Department of Health (FDOH) has implemented Florida Health Connect, a patient portal, that provides clients access to their personal health record. Clients use Florida Health Connect to manage all their health information, from all clinicians, in one place. The portal enables individuals to easily and securely collect, store, manage and use their personal health records and wellness information on a voluntary basis. An individual’s personal health information is only accessible by the individual client or those they designate.

2 Signing Up

2.1 Why sign up for Florida Health Connect?

Florida Health Connect is a patient portal that provides clients access to their personal health record. The portal is an untethered personal health record, which means it isn't offered by a health provider or health insurer but is created for your use and under your control. This is important if you get care outside your primary care network, move, or change health plans, because your health information can go with you. Second, Florida Health Connect is there for your access only and you control how your information is used and who has access to it. It is a private and secure place to collect, store, use, and manage your health information. The online personal health record helps you gather and use all your health information. Using Florida Health Connect, you can easily collect data from doctors, hospitals, and labs, or add data of your own, communicate securely with clinicians at the county health department, and use all the information to manage your health.

2.2 I view myself as a healthy individual, why should I sign up and use Florida Health Connect?

There is an advantage to having your annual checkup information, or any care you may receive for an ailment, from any clinician, in a single location for reference over time as your health situation changes. For example, in an emergency, it is helpful to have your health information - that you have reviewed - immediately accessible to a medical professional. In addition, the tool has the capability to capture information from wearable devices to better connect your health and wellness information gathered outside of the healthcare system.

3 Account Creation

3.1 What do I need to create a Florida Health Connect account? Internet access from a personal device and an activation code provided to you by FDOH. 4 Username and Password

4.1 Are there any special requirements for my username and password? Username

The username must contain between 8 and 30 alphanumeric characters. Special characters are NOT permitted for username, as it is used to create your secure email DIRECT address: ********@******.***.**********.** You may create a username (testAshley1). However, this username will be translated to all lower case letters for the purpose of auto-generating your DIRECT address: ***********@******.***.**********.**

Password

The password must contain between 6 and 20 characters and at least one each of upper-case letters, lower-case letters, numbers. The following password example is ten characters and contains a special character, upper- and lower-case letters, as well as numbers: Spr!ng2019

4.2 Are username and password case sensitive?

The username is not case sensitive. If your username is testAshley1, you may enter all lower case, upper case, or mixed case. However, the password is case sensitive. 5 Records Management

6 Security

6.1 I worry about the security of my health records, is Florida Health Connect secure? Florida Health Connect’s data is stored and accessed through a federally-compliant health IT server, which will be hosted and managed by a leading health information technology provider. Florida Health Connect utilizes several levels of security and encryption to protect your health information while it is stored in your private account and when sent over the internet between secure systems. 7 Privacy

7.1 My health records are private, how do I know that no one will be looking at my health information?

Those who set up a Florida Health Connect profile will remain in sole control of who has access to their account. No one has access to any private health information in a user’s Florida Health Connect account unless the user decides to share it.

7.2 Will you disclose my health information to third parties? The Florida Health Connect application will not share your private health information unless you initiate the sharing. Information is shared only when you give consent, so you determine who sees your health information and for what purpose. We are providing this tool solely to help you manage your healthcare.

7.3 Will you mine my health data to sell to advertisers? We will never mine your data or sell your data to advertisers. We are providing this tool solely to help you manage your healthcare.

8 Personal Health Record (PHR)

8.1 What is a personal health record or PHR?

A personal health record is a summary of health-related information – from sources such as physicians, hospitals, labs, or devices - that is solely controlled by the individual user/patient or a representative of a patient with the authority to access such data. Such access by a representative may include but not be limited to those granted permission by the patient that controls the record. 8.2 What is an EHR and PHR and what is the difference? The generally agreed upon definition of these terms relates mainly to the ownership of the data. Once data is in a PHR it is usually owned and controlled by the patient. Most EHRs, however, are the property of the provider, although the content can be co-created by both the provider and patient. In most states, a patient has a legal right to request their healthcare data and health care providers using a certified EHR will be required to provide an electronic copy as well. An EHR, or Electronic Health Record, is a digital collection of patient health information compiled at one or more visits in any care delivery setting. A patient's record typically includes patient demographics, progress notes, problems, medications, vital signs, past medical history, immunizations, laboratory data and radiology reports and images. The term EHR is often used to refer to the software platform that manages patient records maintained by a hospital or medical practice. It is also used to distinguish from the legacy paper record and contains the same information, only in electronic form. A PHR, or Personal Health Record is a health record where the patient maintains their health data and the information related to their care. It can be stored in an electronic portal or application like Florida Health Connect that patients can use to manage and maintain their healthcare records. Health information can be populated and organized by patients. When contained in an electronic portal or application, PHRs are typically fully accessible at all times online by the patient and are patient-owned.
