This letter is to acknowledge your separation from the company and to provide you with important benefits-related information.

Medical/Dental/Vision Coverage

If you were enrolled in a Company-sponsored medical, dental, and/or vision plan, your coverage will cease as of the end of the month in which you no longer satisfy the eligibility requirements for a particular plan benefit. You and your eligible dependents may be eligible to obtain continuation coverage for a period of time thereafter pursuant to the Consolidated Omnibus Budget Reconciliation Act of 1985) (“COBRA”). The COBRA coverage offered will be based on your enrollment at the time of your separation. During the initial COBRA enrollment election, you cannot switch or elect new plans or coverage but you will have the option to decline current plans or remove eligible dependents. Opportunity to change plans would only be available during the annual open enrollment period. Further details regarding COBRA coverage, including rates and related forms will be sent to you under separate cover from Mercer Marketplace 365+, the Company's administrator of COBRA benefits. You should receive this information approximately 2-3 weeks from the date your separation is processed by the Company. Mercer Marketplace 365+ can be contacted at 1-866-***-****. Please note: If you or a covered family member are eligible for Medicare and choose to elect COBRA, the COBRA coverage becomes the secondary insurance to Medicare regardless of whether the participant has enrolled in Medicare coverage. Thus, the claims will be adjudicated as if Medicare coverage exists, and the COBRA plan will only reimburse you for covered services that Medicare did not cover. Please refer to the COBRA paperwork for any penalties associated with not enrolling in Medicare timely. Also, per the Affordable Care Act, there may be other medical coverage options available to you and your family through the Health Insurance Marketplace. You can access www.healthcare.gov for more information on the health care coverage available through the Marketplace.

Flexible Spending Account

If you were participating in the Health Care and/or Dependent Care Flexible Spending Account (FSA), your FSA debit card will be deactivated effective on the day following your last day of employment. If you have a Health Care FSA and your payroll contributions were greater than the expenses you incurred during the period while you were participating in the Health Care FSA, you may wish to continue contributing for the remainder of the plan year on an after-tax basis under COBRA.

Health Savings Account

Health Savings Account (HSA) participants will be able to keep their account and any funds in it. It is important to note that you may use HSA funds to pay for health insurance premiums while receiving unemployment

(including COBRA continuation coverage). However, you will not be able to contribute into the account if you are not covered by a HSA-eligible healthcare plan. You have 3 options regarding the funds in your HSA:

- Leave them in the existing account with Mercer Marketplace 365+ (please note that a monthly administrative fee will be applied to your account)

- Transfer your HSA money to another custodian or trustee

- Withdraw your HSA money (may be subject to tax and penalty). Important information regarding Mercer Marketplace 365+: your current Mercer profile uses your work email address, so it is critical that you log in (accounts.mercermarketplace.com) and update your contact details to receive important documentation. We recommend you perform this update within a week of your termination date. Please contact Mercer Marketplace 365+ at 1-877-***-**** to obtain further information. Life Insurance

If you were eligible for company-sponsored life insurance coverage, it will cease on your last day of employment with the Company. You may be able to convert all or a portion of the coverage that you had while employed into an individual policy. Lincoln Financial Group will automatically mail you a Conversion and Portability packet shortly following your separation, which includes instructions and contact information if you have questions. A completed application must be submitted to Lincoln Financial Group within 31 days of your termination from the Company to convert the coverage. Group Legal Plan

If enrolled, your participation in the Group Legal Plan (the "Legal Plan") will cease on the last day of employment with the Company. You may continue coverage by contacting MetLife Hyatt Legal within 30 days of your separation date. The length of portability is 30 months and must be paid in full directly to MetLife Hyatt Legal. To apply contact MetLife Hyatt Legal's Client Service Center at 1-800-***-****, Monday-Friday (8am- 7pm ET). Commuter Benefits

Participants in the Commuter Benefit Program through the Health Equity WageWorks Commuter Transit Card will have 90 days to spend the remaining funds on your Commuter Card. At the end of the 90-day period, any post-tax funds remaining on the card will be returned to you and any pre-tax funds will be forfeited. Cards used for Commuter Parking will be deactivated immediately after you separate from the Company. Any post-tax funds remaining on the card will be returned to you and any pre-tax funds will be forfeited. Please note that you are not required to return transit passes upon termination. If you do, only post-tax funds will be returned to you.

Please update your email address at Health Equity WageWorks by logging on to www.wageworks.com so you will continue to receive correspondence from Health Equity WageWorks. Retirement Benefits

When you leave the Company or its affiliates, you are entitled the vested value of your account under Internal Revenue Service code and the Altice USA 401(k) Savings Plan and Cablevision Cash Balance Pension Plan (if applicable) plan rules. Further information is available at the plans’ record-keeper, Fidelity. You may choose from February 2024 - Standard Page 2 of 5

the following options:

- Receive a distribution in a single lump sum payment paid to you; or

- Direct rollover to another qualified Plan or IRA; or

- Receive installment distributions - one distribution per year over a period not to exceed 15 years (Altice USA 401(k) Savings Plan only); or

- Leave your benefit in the Plan until you request a distribution and in accordance with Plan rules

- Receive a life annuity (Cash Balance Pension Plan only)

- If married, receive a 50% or 75% joint and survivor annuity (Cash Balance Pension Plan only) When your employment with the Company ends, if you have a loan from your 401(k) plan account with an outstanding balance, the length of the repayment term will remain unchanged. You will be responsible for setting up the payment schedule and method directly with Fidelity. You should receive the detailed information on your options from Fidelity within 4 weeks of your termination. Note: If you do not repay any outstanding loan balance by the agreed repayment schedule, the remaining balance will be considered a "deemed distribution". You should consult with your tax advisor as to the tax consequences of such an event. Please consult a tax advisor when taking a distribution to consider full tax implications.

Fidelity Personalized Planning & Advice (PPA)

Participants in PPA will continue unless you elect to cancel your participation. To cancel, please contact the Altice USA Service Center at Fidelity at 1-877-***-****.

Employment Verifications: The Work Number

Should you need to verify your employment history with the Company, please refer the requester to The Work Number, our external verification service designated to provide automated employment and income verifications. Contact them at http://www.theworknumber.com or call 1-800-***-****. You will need to provide the verifier with your SSN and the Company employer code: 36104. Employee Product Benefit (EPB)

If you have separated as a result of a resignation or termination of employment, the EPB ceases on date of separation and direct billing will commence thereafter, at current retail rates. If you were separated as part of a layoff, reduction in force or an elimination of position, the EPB continues for 30 days from the date of separation. Following the 30 days of extended benefits, direct billing will commence thereafter, at current retail rates.

If you voluntarily resign from your position, are part of a layoff, reduction in force or an elimination of position and have 20 or more years of continuous years of service, you may be eligible for extended benefits. To confirm your eligibility and for the duration of the benefits, contact the Employee Accounts group. Although the Employee Accounts group will be notified of your separation, you should request any changes in services or update your account by contacting Employee Accounts at 516-***-**** or at *********@*********.***.

Payroll – Final Pay

If applicable, direct deposit will remain in place for all remaining pay from Altice USA. Final pay would include last day(s) worked, time off taken within that period, and any unused/accrued vacation hours. If state guidelines require payment of other time off or vacation hours to be paid with final pay, you will receive payment with your final pay.

Post Separation Commission Payments

If you were eligible for commissions as of the effective date of your separation from employment under an applicable Company commission plan, any payments you may be eligible to receive after the date of your separation of employment will be processed for payment in accordance with the terms of your applicable commission plan in the second scheduled commission pay period following the separation date (approximately 60 days after the date of separation) or as otherwise may be required under the terms of your commission plan and/or applicable law. There may be unique circumstances that may warrant additional time to calculate commissions to determine amounts earned; in which case any final commission payment(s) will be processed once earned commissions have been calculated by the Company. Pay Statement and W-2 Access

Former Altice USA employees can access and print pay statements and W-2s by accessing the ADP iPay site. Log onto https://ipay.adp.com. You may need to re-register to access the iPay website. The registration pass code is csccorp-ipay. If you have previously registered using your work email address, you may need to have the email updated to a personal email address. Please contact the HR Service Center. Post-Employment Confidentiality Obligations

Although you are transitioning from your position with the Company, the terms of the Confidential Information Policy, Intellectual Property Policy and the Code of Business Conduct and Ethics Policy cover the period of your employment and continue indefinitely. Consistent with your ongoing confidentiality obligations, please coordinate with your HR Representative and Manager for the return or removal, where appropriate, of all electronic or other documents and materials including copies that you created or received from the Company during your employment (i.e. soft and hard copy data, and any drafts). This extends to any personal devices upon which you may have any Company confidential information. You will also be required to surrender all electronic devices provided by the Company during your employment. You are not permitted to share any confidential information, intellectual property, or other company business information with third parties at any time without the prior written consent of the Company’s CEO or CFO.

Unemployment Insurance Benefits
