oMeet with patients/family in the Emergency Department as they are being admitted, and discuss discharge planning needs based on current medical history, patient/family input, and work-up

oProvide case management coverage for Bethesda North Hospital as point of contact once day shift staff leaves

oResponsible for covering Bethesda Butler Hospital, Bethesda ArrowSprings for any care management needs after day shift staff leave the facilities

oResponsible for covering all inpatient units at Bethesda North for discharge needs, Medical Ubers, any Ambulance transport needs, equipment needs, as well as setting up home health services, home PT/OT, IV therapy, etc.

oDiscuss with patient/family regarding questions for any drug and alcohol treatment, and/or mental health facilities, and arrange transportation to facilities if needed

oCall and arrange transportation for patients who require immediate psychiatric placement, and/or AirCare transportation for critical care treatment

oWork with local vendors to provide patient’s with needed durable medical equipment at the time of discharge, (i.e., walker, rollator, shower chair, oxygen, wheelchair, etc)

oWork with local skilled nursing and/or rehab facilities to help place patients as needed, once evaluated by PT/OT staff

oInitiate precertification process for placement in Inpatient Rehab, Long-term care placement, etc., for Medicare Advantage Plans, State Medicaid, and/or private/commercial insurance companies

oWork with the emergency room physicians, hospitalists, as well as the mid-level providers, to ensure patients can be discharged home safely

oCall other hospitals/facilities to make arrangements to safely transport patients to their location for more definitive care. (ie., Air Care, ACLS, and/or BLS squad)

oUse inhouse interpreter service to ensure all patients have their medical treatment, and discharge needs discussed with them in their own language, to help patients be more successful at the time of discharge
