Supervise both FTE NA’s and CNA’s and well as PRN staff. Responsible for assessing residents for safety, med compliance, ADL’s and IADL’s, companionship, sitter services and respite care needs. Also provide private pay housekeeping, shopping, and accompanying to physician offices for services. Responsible for policy and procedure writing per CMS, Quality Assurance, Budget writing. Responsible for staff in service and education, hiring process and payroll. Responsible for implementing less staff turnover and handling any resident or family grievance or complaint.
Also responsible for instructing staff on BLS certified through Blue Ridge Community College.

Developed 2 new programs from the ground up. Ambulatory Wellness and Transition of Care. The
Ambulatory Wellness Program is MCR driven that is no cost to the patient and all revenue to the Provider. Patients are covered to see RN once annually to do a comprehensive assessment identifying social determinants, medication review and education, ADL’s and IADL’s, providing and updating vaccinations, mammograms, colon screening, AAA, Dexa Scans, safety and fall risk assessments, depression and pain screenings, BMI and nutrition education, vital signs. After visit RN is responsible for collaborating with physician and any interdisciplinary needs such as SW, HH, DME etc. and obtaining order from physician and follow through. RN’s saw 9 patients per day for 45 min comprehensive visit.
Transition of Care program was for inpatient hospital follow up. Again, this is no cost to patient and only revenue for provider. Patients are called within 48 hours of being discharged home. Patients’ med list is reviewed, and any problems identified are done so at this time such as meds not being at pharmacy, cost too expensive, transportation issues to obtain meds, etc. Patient is made hospital follow up appt and if need to have other disciplines ordered is done so at that time such as HH. Responsible for all hiring process and payroll.
I was responsible for the initial roll out on March 16th for the COVID testing center at Pardee with collaboration of others such as our Infectious Disease MD, CDC and DHHS. Me and my team head fronted the testing, vaccinations, and call center for 6 months thereafter until other staff could be trained and incorporated with BRCC for space for community vaccination.
I was the only RN to begin seeing patients when the program started and prior to leaving had a staff of 5 FTE RNs located in 7 of the Provider offices, 2 CNA’s and 1 CSR. Both programs are successfully thriving today.
From 2016-2018 still at Pardee-RN Case Manager-RN case manager in Home Health responsible for patients of all levels in home providing nursing care, social work assessments, wound care, IV’s, medication education, diabetes education, preceptor to new hires as well as college students. Helped implement the first EMR for Home Health which was EPIC.

Responsible for writing policy and procedures, supervising all CNA staff, ensuring annual competencies, payroll, hiring process. Admission for Title III patients in home and billing. Supervisory visits to CNA’s and along with patient and family developing a Plan of Care to keep patient in home for as long as possible. Working with Hospice or end of life transfers. Also provided services to private pay as well as Title III.
In 2006 at CarePartners-Developed the first Pediatric program for Home Health in Henderson,
Transylvania, Polk and parts of Buncombe counties. Responsible for newborn care through age 18. Regular nursing assessment and education for pediatrics as well as transitioning parents to home with complicated pediatric babies who may have tube feeding, apnea monitors, bilirubin lights and just got out of PICU.
