DEFINITIVE HEALTHCARE, LLC is delighted to extend an offer to you for the position of Data Entry Clerk (Remote) within our esteemed organization. We are highly impressed with the potential you bring to our company and are excited to have you onboard.
As previously discussed, your work will be remote until the local branch is commissioned and you are willing to work on-site. You will be entitled to an hourly salary starting at $29 per hour, which accurately reflects the cost to our organization. You will be on probation for one month, during which regular performance reviews will be conducted to assess your performance and suitability. Your continued employment is dependent on your successful completion of the probationary period. After six months, your salary will be reviewed, and subsequently, every 12 months. You will be entitled to all allowances and benefits decided by our management.
By accepting our offer of employment, you acknowledge that your employment will be on an at-will basis. Neither you nor any company representative has entered into a contract regarding the terms or the duration of your employment. As an at-will employee, you have the freedom to terminate your employment with the Company at any time, with a cause or advance notice. Similarly, the Company reserves the right to reassign you, modify your compensation, or terminate your employment at any time, with advance notice.

Probationary Period:
Following the requirements of your role, a probationary period of one month shall be applicable. During this period, you shall be remunerated at an hourly rate of $29 and shall receive the necessary training and guidance to ensure that you are proficient in your designated responsibilities.
Your appointment shall be contingent upon the satisfactory completion of the aforementioned probationary period. Termination of the probationary period may be effectuated by either party, upon one week's written notice, or through payment instead of notice.
Payment:
It is hereby notified that, as an employee, you will receive a remuneration of $29 per hour, which will be disbursed weekly or bi-weekly, based on the hours worked during that particular week. It is expected that you will work for a duration of 6-8 hours per day and a total of 20-40 hours per week. We offer you the flexibility to choose a schedule that suits your preference, provided that your daily responsibilities are fulfilled. Additionally, any expenses that you incur while executing your duties on behalf of the company will be reimbursed expeditiously, either immediately or during your respective pay period.
Superannuation:
Superannuation payments will be made on your behalf by the Superannuation Guarantee into a Superannuation fund of your choice. If you do not choose a fund, your contributions will be placed into the default fund.
Salary Review:
Personal leave:
Your remuneration will be periodically reviewed, with the first assessment after six months and subsequent reviews annually or according to workplace laws. Reviews may also be on or around your employment anniversary. Please take note of these review periods for your career development and financial planning. Contact your supervisor or HR representative for any questions or concerns.
As per the guidelines set forth by The Fair Labor Standards Act (FLSA), you are entitled to up to 10 days of paid personal or career leave. Moreover, you have the right to avail up to two days of unpaid career leave for each occasion, as per the FLSA. Additionally, in the event of compassionate leave, you have the right to take up to two days of paid leave per occasion under the FLSA. It is imperative to note that the aforementioned provisions are by the FLSA, and it is your right to avail of them.
In order to ensure a smooth transition when terminating employment, employees are required to provide their employers with a minimum notice period. This notice period can be determined by one of two methods:
• The period of notice defined in the employee's employment agreement or an alternative contract of employment.
• If no notice period is specified in the aforementioned agreement, the minimum notice period is equal to the employee's typical pay period.
It is advised that employees adhere to these notice periods, to maintain a professional relationship with their employer and to ensure a smooth transition out of the company.

DEFINITIVE HEALTHCARE, LLC may terminate your employment by giving the following period of notice (or payment in lieu) depending on service:

If you are over 45 with more than two years of continuous service, you will receive an additional week’s notice (or payment in lieu).
The company may terminate your employment at any time without notice if:
You are guilty of serious misconduct; or you are in material breach of a provision of this contract, including confidentiality undertakings.
Following the termination of your employment, you will be required to return all company property.
You are required to observe and uphold all the Company’s privacy policies and procedures as implemented or varied from time to time.
Collection, storage, access to, and dissemination of employee personal information will be by privacy legislation.
