I hereby provide consent to Knight-Swift Transportation Holdings, Inc., and its agents, subsidiaries, and assigns, including, but not limited to Swift Transportation Co. of Arizona, LLC, Kold Trans, Barr-Nunn Transportation Inc., Abilene Motor Express Inc., and Knight Transportation, Inc., (the "Company"), along with its Third-Party Administrators ("TPAs"), DriverFacts and Tenstreet, on the behalf of Company, to conduct a limited query of the FMCSA Commercial Driver's License Drug and Alcohol Clearinghouse (Clearinghouse) to determine whether drug and/or alcohol violation information exists about me in the Clearinghouse.
