Skills
Obtain and document vital signs, height and weight Observe patients for physical/mental status or behavioral changes and reports changes to licensed personnel
Communicates pertinent information with members of the healthcare team Measure and records input and output as ordered
Assist patients with ADLs as needed
Turn and reposition patients as needed
Change linens of occupied and unoccupied beds
Assists with admission, discharge and transfers of patients via bed, stretcher, and wheelchair
Ensure trash and biohazardous material are emptied into the proper receptacle Empties and measures drain output (percutaneous, hemovac, ileostomy, colostomy, and NGT canisters)
Assist with pre-op surgery baths
Applies restraints according to facility protocol
Provides foley catheter care as needed; discontinue foley catheter as ordered. Provides 1:1 supervision for demented and suicidal/homicidal patients Provides incontinent care as needed and applies incontinent and barrier creams as needed
Monitors patient's skin for issues and reports to licensed personnel Responds to nurse call and assisting patients with needs Prepares isolation rooms and provides supplies per infection control guidelines
Initiates code blue and administers effective chest compressions Assists with post-mortem care
Instructs and trains co-workers
Willing to work overtime when needed.
