Senior Care Services (15 nursing homes, 7 medical directors, > 45 FTEs and 4,500 lives managed)

The UPMC Regional Vice President is responsible for staffing, operational and financial performance, quality metrics, strategic vision/development and patient experience of his/her programs. Direct oversight of all budgets/contracts/recruitment/terming and staff development including co-chair of the Value Implementation Team. Responsibilities include: seven hospitals, 8 - ICUs, nine CCRC/SNF/rehabs, six clinics and > 30 medical directors with 500 + FTEs.

Achievements:

Development of an employed Pulmonary/Critical Care program from 2019 – 2020, recruiting over 25 physicians and 20 APPs in 14 months - just prior to COVID 19 pandemic - while moving on our previous private pulmonary and critical care group (one of our biggest team achievements)

Personally worked in our ICUs and nursing homes 2020 - 21 (during pandemic surges)

Development of three additional Pulmonary practices 2019 – 2021

Development of Sleep Medicine services as an independent entity 2020 -2021

Reduced locum utilization at Legacy hospitals in 2015 to zero over two years with a savings annualized at 6 million dollars.

Expansion of ICU APP program across the system, 20 FTEs, with zero locums, savings of 1.8 million dollars and eventual build of a system employed Pulmonary Critical Care Program

EPIC Hospitalist and ICU support of successful go-live in five new hospitals, 2017 - 2019

Development of a Hospitalist Heart Failure service line – 2017-present, success with readmission rates of 3-5 %, LOS reduced X 1 day CPC reduced X 1200 dollars, Truven mortality 1%, received the HAP 2019 Excellent Care Award for large healthcare systems and AHA conference presentation

Brokered extraction of 20 Hospitalist providers in Lancaster region, savings of 2 million dollars/year

Real readmission reduction as noted in PHC4: COPD readmission rate reduction from 13 % to 3.6% to 2.8 % over three years. Sepsis to sepsis readmission rate at 15.6 compared to state average of 18%. CHF team readmission reduction from 16 to 5 % over 22 months and increased palliative care from 20% to 90%, SNF sepsis readmission rate down from 19% to 14% 2016-2018

First nationally, to develop a shared savings agreement with Continual Care Retirement Communities (CCRC) setting a new standard for SNF/rehab care and reimbursement

As System Sepsis Chair, reduced severe sepsis mortality over 12 months by 40 % consistently through establishment of a 25 member Sepsis CET ranking PHS in the top 10 % in the nation for sepsis care as ranked by CMS, also increased three hour sepsis bundle compliance from < 50 % to > 85% for that same time period through collaboration, standardization with ER/Hospitalist and Nursing. Reduced sepsis LOS by 0.6 days saving approximately 3 million dollars

As System CMI champion, saw a system CMI bump from 1.41 to 1.57 and CGOH up from 1.44 to 1.62

Developed and directed an In Patient Liaison Team with our Director of PACN, mentoring APC’s through a population health model and on call for sepsis reducing readmissions from 33% to 12 % through real time collaboration

As Associate Medical Director and Chair of Department of Medicine, was directly responsible for: Onboarding employed Hospitalist and thereby completely reducing Locums use to zero within one year, savings realized 1.5 million. Worked closely with non-employed per diem staff to achieve this objective through comp plan retooling, APCs and friendships

As Chair of System Pneumonia CET (over a 5 year period) Achieved 95% + metrics with antibiotic timing, vaccination and blood cultures. During that same tenure saved the system > 500 K per year in collaboration with ID and Pharmacy to facilitate automated antibiotic substitution and promote stewardship. Reduced C. diff infection rates X 35 % through quinolone reduction and hygiene campaigns for all - ongoing
