Dear Sara,
On behalf of Q-Services, LLC and Metronet Holdings, LLC, I am pleased to offer you the at-will Full-Time position of Care Executive. In that role, you will report to Mark Gosse. Below is a summary of the proposed compensation package for this position.
Hourly Compensation - $17.00 USD an hour paid on a bi-weekly basis for time worked. Any time worked over 40 hours per week will be paid out at the rate of 1.5 times your regular hourly rate.

Commission - Commissions will be paid out according to the Customer Care Associate commission plan (a copy of which is attached for your review).

Paid Time Off (PTO) - You will accrue PTO at the new associate rate of 3.08 hours per pay period, which is the equivalent of 80 hours on an annual basis. Your PTO accrual will increase with seniority according to the PTO accrual schedule described in our Associate Handbook, as revised from time to time.

Paid Holidays - Paid holidays vary from year-to-year depending on weekend days, but generally include Christmas, New Years, Memorial Day, July 4th, Labor Day, Thanksgiving and two additional holidays.

Health Benefits - You will be eligible to apply for medical, dental, vision, life, and disability insurance on the 1st of the month following one full month of employment. If your start date is the 1st or 2nd of the month you will be eligible for all insurance plans the first of the month following 32 days of employment (a benefits summary is attached for your review).

401(K) Eligibility - You will be eligible to enroll in our 401(k) plan at the next quarterly enrollment period following your 5 month anniversary.

Metronet Value Plan “MVP” – You will be eligible to participate in our Metronet Value Plan based on eligibility criteria outlined in the attached FAQ document. After you have met the tenure requirement, the exact number of shares you will receive will be determined and communicated to you accordingly.

Our offer is contingent upon the completion of a successful background investigation which involves a review of your criminal background and motor vehicle record, drug screen results and any other information we deem reasonably relevant to your candidacy. A criminal conviction is not an automatic bar to employment with us. As a condition of your employment, you will also be required to formally acknowledge and agree to abide by our employment policies and procedures and verify that you are legally- required to work in the U.S.
