People with disabilities are protected by the Americans with Disabilities Act of 1990 and Section 504 of the Rehabilitation Act of 1973. Students with disabilities who attend Southern Union State Community College are entitled to reasonable accommodations under these laws. LaNia Greathouse has a documented disability. The following accommodations would be appropriate:

• Allow preferred seating, if requested. (Near the instructor)

• Assist student by arranging the services of a volunteer note-taker if requested or provide instructor’s notes, if requested.

• Student will use assistive technology for notetaking support (i.e. OneNote, Glean, cell phone—for taking photos of material on the whiteboard during lecture, etc.)

• Allow student an alternative method of assessment instead of an oral presentations in class.

• Allow student to record lectures.

• Allow student to use a calculator when completing math test, quizzes, and assignments that are designed to assess student's procedural knowledge rather than computational skills.

• Provide copies of PowerPoints used in class.

• Oral testing, if requested.

• Allow student double time on exams, quizzes and additional time for in-class assignments.

• Allow student to take tests in a distraction-reduced setting, if requested. Please note that the above accommodations are not designed specifically to meet this student’s needs in your class. The degree to which the student needs accommodations may vary with each class and exam format, and your teaching style. The student may even feel that some accommodations are not warranted in your class; other accommodations may need to be clarified. For example, the amount of extra time will depend upon the type of test and the degree to which the student’s disability may impact. RE: Procedures for Providing ADA Accommodations

You are receiving this form because your student has requested academic accommodations for this semester. Here are a list of procedures to assist you with providing accommodations for your student. 1. It is the students’ responsibility to notify the instructor that they have been provided accommodations for the semester.

2. The student will email ADA documentation to the instructor and will also need to meet (via email, virtually face-to-face) with the instructor to determine how the accommodations will be carried out.

3. Instructors should email the student to confirm receipt of the accommodations and the agreement on how the accommodations will be carried out for the semester. 1. It is the students’ responsibility to notify the instructor that they have been provided accommodations for the semester.

2. The student will email ADA documentation to the instructor. 3. The student will schedule a meeting with the instructor to discuss how accommodations for the semester will be carried out (ex. testing accommodations, captioning service, note taking, etc.). Test Taking Accommodations

1. Accommodations for additional time for testing and quizzes in Canvas a. Instructors will be responsible for setting this time parameter. 2. Accommodations for additional time for testing in ProctorU a. Students with accommodations are allowed their additional time. b. Email all accommodation information to ad5ntw@r.postjobfree.com. c. Please contact Nancy Bramlett (ad5ntw@r.postjobfree.com) or Beth Barks (ad5ntw@r.postjobfree.com) for assistance in setting up testing for students with additional time. Providing Documents for Instructors for Virtual/Online Courses Additional Information for Virtual/Online/Hybrid/Traditional Courses Providing Documents to Instructors for Traditional Courses 3. For traditional courses taken on campus, instructors are responsible for providing the testing material for the Assessment Center (Business & Technology Center (BTC), Room 109) if this is the designated place for testing. Please contact Ms. Debra Todd (ad5ntw@r.postjobfree.com) for more information on setting exams in the Assessment Center.

a. Instructors will need to complete a Faculty Request for Proctored Test Administration form and forward the form to Debra Todd (ad5ntw@r.postjobfree.com).

b. The instructor will also need to forward the testing documents to Assessment Center as well as the testing document.

c. The instructor will need to set up the testing appointment in the Assessment Center at least a week in advance.

d. If the instructor prefers to proctor the exam themselves, that is also an option. 4. Faculty must provide the same format assessment as used in the classroom. Faculty may not require a student to complete an alternative format of a test as a result of scheduling changes related to accommodations unless the change relates to an ADA testing accommodation. 5. The student is responsible for arranging timely makeup exams or quizzes and completing missed assignments within the timeframe established by their instructor. Accommodations for oral testing for traditional/virtual/ online/hybrid courses: 1. Instructors can record their tests for students requesting oral exams in Canvas. Click here for instructions. This option is available for both traditional and online students. 2. If a Canvas recorded exam is not a viable option, please refer students to Cydney Mathews

a. Students will be required to make an appointment for testing and she will provide the oral testing for the student. Oral tests are only reading questions and response options—not explanation, expounding, or additional help with responses.

b. Instructors are responsible for providing testing information and material to Mrs. Mathews via email or courier.

Extended deadlines and missed exams (disability-related) 1. If you have a student who has an accommodation for extended deadlines for missed assignments or tests due to disability-related reasons, there is a guideline of up to 7 days to complete the missed assignment. It is at your discretion to determine an appropriate length of time up to 7 days to complete their assignment. This deadline should be set during the initial consultation after the instructor receives the accommodations. 2. The student will need to request an extension prior to the assignment’s due date. 3. The student is responsible for contacting the instructor via email or Canvas within 24 hours (if possible) to notify them of disability-issues which caused them to miss a test or assignment. 4. If there is an issue, please contact Cydney Mathews at ad5ntw@r.postjobfree.com to discuss. Attendance Accommodations

5. Excessive/Habitual and non-disability related absences are not covered by accommodations. 6. Students are required to notify the instructor within 24 hours (if possible) of class meeting time if the absence is disability-related. Communication with the instructor is very important. 7. The student is responsible for arranging timely makeup exams or quizzes and completing missed assignments.

8. Excessive absences should be addressed with the ADA Coordinator. Note Taker or Instructor’s Notes Accommodation

1. If a student request this accommodation, the Instructor will assist by asking for a volunteer note taker in the class who would be willing to share notes with another student. 2. Please keep the identity of the student requesting the notes anonymous. 3. The volunteer note taker will provide copies of the notes to the Instructor electronically or by hard copies and the instructor will provide copies of the notes to the accommodated student. a. If a volunteer isn’t available, please provide any instructor’s notes, if available. b. If a volunteer note taker or instructor’s notes aren’t available, encouraging students to make an appointment during office hours to help the student fill in any gaps in note taking is also be a suggestion.

Student Requests Accommodations Not Listed on the Accommodations Memo Instructors are only responsible for providing the accommodations listed on the Accommodations Memo from the ADA Coordinator and if a student requests an accommodation that is not listed on the listed, refer the student to the ADA Coordinator for further assistance. Accommodations can be provided to the instructor at any time during the semester, however, they are not retroactive and are not in effect until the instructor has received a copy.
