EXPERIENCE

Health Alliance Plan 3/2018 – current

I work with a population of members that are at the highest risk for major health concerns. I provided telephonic and in person assessment, with the input of the member I assist with developing a plan of care. The plan of care includes the member’s goal, the physician, any other health care member that can assist with the goal, teaching, providing necessary resources, additional services such as paid caregiver, home delivered meals, non-medical transportation, lawn, and snow removal.

Molina Healthcare, Care Coordinator 9/2015 to 3/2018

In my role as a care coordinator, I provided direct and telephonic case management with a complete health risk assessment. I assisted with providing members with health management via teaching and community resources, developing a plan that will help meet their health care and psycho-social needs.

I provided start of care visit to patient from Veteran Hospitals that are either clinic or hospital discharge.

I provided in home case management for patients to assist with care. I provided this to patient via teaching, hands on care, providing additional staff and resources into the home with the use of a multi medical team approach. The team included doctors, therapist, medical social workers, family member per the patient needs and request.

Other Qualification and Experience

Knowledge of medical/health insurance terminology, ICD-10/ICD-11, HIPAA and DOL regulations.

Proficiency with Microsoft Office products (Word, Excel, Outlook, etc.).

Strong verbal and written communication skills.

Experience using video conferencing.

Excellent customer service attitude, accuracy, and professionalism.
