2)Sanitation & Sewerage Sub-project: It involves detail survey, design and construction supervision of three nos. of combined sewerage system, there are three schemes name as Rapti-1, Rapti-2 and Karra system of 13km length with 1500mm to 300 mm dia. RCC hume pipe with Waste Water (reed bed) treatment plants each on the banks or Rapti and Karra rivers serves wards 1, 2, 4, 5 and 10 of Hetauda Municipality. The sub-project includes construction of 3 nos. of sewerage system. The total sub-project cost is Rs. 145,080,402.83
3)Supply of Sewer Cleaning Equipment Sub-project: There are sewer cleanings equipment like jetting machine, suctions machine, di-silting machine, 2 nos. of tripper, one no. of tractor etc. The total sub-project cost is 17,495,878.14
4)Waste Water (Reed Bed) Treatment Plant Sub-project: It involves detail survey, design and construction supervision of three scheme of Waste Water (Reed Bed) Treatment Plant sub-project name as Rapti-1, Rapti-2 and Karra system. The total sub-project cost is Rs. 42,124,197.65
5)River Training Works Sub-project (Rapti and Karra River Bank Protection Work): It involves detail survey, design and construction supervision of river embankments to protect against flood, erosion, from Rapti & Karra rivers in wards 1, 11 & 4 respectively. The total project cost is 9,168,350.37

Activities Performed: Construction supervision of project, review quality assurance plan and quality control plan (QA QP) supervise and monitor construction, review contractors bill (IPC), monitor and enforce safety manual, monitor physical & financial progress, examine contractor's claim, check installations and commissioning, monitoring preparation of as built drawings. The project was implemented and managed under ICB contracts; Project Cost: NRs. 10 millions.

I, the undersigned, certify to the best of my knowledge and belief that
(i)This CV correctly describes my qualifications and experience;
(ii)I am not a current employee of the Executing or the Implementing Agency;
(iii)In the absence of medical incapacity, I will undertake this assignment for the duration and in terms of the inputs specified for me in Form TECH-6 provided team mobilization takes place within the validity of this proposal;
(iv)I was not part of the team who wrote the terms of reference for this consulting services assignment;
(v)I do not have conflict of interest in accordance with ITC 3.0;
(vi)I am not subject to any national or international sanctions, temporary suspension or debarment by ADB or other multilateral development banks (MDB);
(vii)I certify that I have been informed by the firm that it is including my CV in the Proposal for the Design, Supervision and Contract Management Consulting Services (DSC/WW/03) for Kathmandu Valley Wastewater Management Project. I confirm that I will be available to carry out the assignment for which my CV has been submitted in accordance with the implementation arrangements and schedule set out in the Proposal.
(viii)I have not been found guilty or convicted of any violation of law that is not a minor traffic violation.
(ix)I can accept payments through the international banking system or otherwise discharge ADB’s obligation upon initiation of wire transfer.
(x)I understand that it is my obligation to notify the Client and ADB should I become subject to any national or international sanction, including becoming ineligible to work with ADB or other MDBs, cannot accept payments through the international banking system, should integrity issues including conflict of interest arise and/or should we or the proposed consultant be convicted of an offence excluding minor traffic violations.
