I have over 18 years of nursing experience that includes 4 years of acute care nursing on a telemetry floor. Recent experience in the Appointment and Advice Call Center as an Advice Triage Nurse. RN Float Pool, Vitas Hospice: July 2021 to September 2021 On call Hospice Nurse assist with end-of-life care and visit patient’s home, asst living, skilled nursing facility or boarding care. Contact Hospice MD and Funeral Homes to pronounce the death of a patient. Assist family and educate concerning patient’s transitioning and when they are imminent. Assist with comfort care and administer scheduled drugs until patient is comfortable.

• Manage hospice patients their medications, DME supplies and provide clinical care; that include wound care, ostomy care, urinary cath, etc. Establish a Plan of Care for hospice patients that provides quality end of life care with multiple disciplines; that include Medical Social Worker, Spiritual Counselor, Hospice MD and Clinical Nurse Manager. ACUTE CARE NURSING SKILLS

• Perform a head-to-toe assessment and Nursing Plan of Care for patients

• Assess and implement measures for adequate oxygenation and gas exchange

• Work at fast paced inbound call center and send electronic messages to their primary physicians if appropriate

• Receive Critical Lab Values that include PT/INR, Electrolytes or Blood Cultures and consult with Call Center Physicians to determine the safest outcome for the patient which can include an ER visit and/or follow up with their Primary Provider with an electronic message

Educate patients with home care advice when appropriate in the Pediatric, Ob/Gyn and Internal Medicine population

HOSPICE NURSE CASE MANAGEMENT SKILLS

• Accomplishes clients’ care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating interdisciplinary approaches and monitoring clinical staff that include CNAs and LVNs
