I understand that it is a crime to knowingly alter, falsify, or fraudulently use this Certificate of Completion in any manner. IMPORTANT CUSTOMER INFORMATION: Completion of a Driver Improvement/Defensive Driving course, in and of itself, will not reinstate your Georgia driver’s license or driving privilege. You may be required to pay a reinstatement fee. In addition, if your Georgia driver’s license or driving privilege is suspended because of multiple or repeated violations, you may be required to pay additional reinstatement fees. If you lose your certificate or if you are in need of a copy, a Replacement Certificate may be obtained from the school you attended for a fee not to exceed $5 or you may obtain a copy from DDS Online Services. You may visit the DDS website at www.dds.ga.gov to check your driving reinstatement eligibility, view, print or email a list of your specific reinstatement requirements, pay fees and reinstate driving privileges online or you may call the DDS Customer Contact Center at 678-***-****. You may also reinstate a suspension in person or by mail. To reinstate by mail, please mail the Certificate of Completion and appropriate reinstatement fees(s) to the Georgia Department of Driver Services, P.O. Box 80447, Conyers, Georgia 30013. Please visit the DDS website at www.dds.ga.gov for additional information and the locations and operating hours of Customer Service Centers throughout the State of Georgia. Attention Students: In addition to reinstating by mail (at the above address), or in person at any of the DDS Customer Service Centers, you may also follow the instructions below to reinstate online.

1) Create or log into your DDS Online Services Account found on the DDS website at www.dds.ga.gov under the Online Services tab. 2) Click Log Into My Account. This gives you the option to either create your account or log in. 3) Click on Defensive Driving & Risk Reduction Certificates. 4) Click on Validate a Certificate of Completion.

5) Enter the Certificate of Completion number, your date of birth, and the PIN # listed at the bottom of this certificate. 6) Click Submit.

a. If a match occurs, the Certificate of Completion will be transmitted to your driving record and a pdf version of your certificate may be printed using the Print a Certificate of Completion option. b. If the system is not able to validate your certificate, a message will display stating “The information submitted does not match the record on file. Please verify the information entered. If the information entered was correct please contact the Regulatory Compliance Division at 678-***-**** or visit a DDS Customer Service Center.” If you do not know your Georgia Driver’s License number or the exam/issue date, you may follow the instructions below to reinstate online. 1) Go to the DDS website at www.dds.ga.gov and click on the Online Services tab. 2) Click Log Into My Account.

3) Click on Certificate Validation in the Limited Services box. 4) Enter the Certificate of Completion number and the PIN #. 5) Click Continue.

a. If a match occurs, a message will display stating “Certificate Validation Successful”. b. If the system is not able to validate your certificate, a message will display stating “The information submitted does not match the record on file. Please verify the information entered.
