Assess individuals, served by the Assertive Community Treatment Team, for mental health disorders and make an appropriate DSM diagnosis. Ensure that services delivered in a medically appropriate manner and care provided follows with service definitions and best practices.

Review past treatments, evaluations, and other relevant information, in order, to develop an appropriate diagnosis and person-centered plans and screens referrals in coordination with the other Assertive Community Treatment team members
