LPN – Urgent Care Clinic
The LPN serves as a responsible member of the Urgent Care Clinic department and are responsible for goals and interventions that promote veterans in attainment maximum functional potential.
Works in the Urgent Care Clinic as assigned under the direction of a Registered Nurse and reports to the Nurse Manager for the Primary Care area.
Responsible for observing, recording, changes in the patient’s condition, as well as providing input into revision of plan and care based upon data collected and identified patient/family needs.
Collaboration and communication with clinic staff, the patient’s family/care giver, internal and community-based services and disciplines involved in providing care to patients.
Assist other staff to prioritize the overall veteran care needs and adjusts the plan of care as needed.
Advanced knowledge and skill in preparation, administration and documentation of commonly prescribed oral, topical, subcutaneous, intra-muscular medications permitted by the facility policies and procedures.
Performs venipuncture for blood draws and IV peripheral insertions. Monitors and adjust rates of IV fluids. Can float to the lab to aid in blood draw rooms as staffing allows. Trained and proficient in the Lab ordering system for collection and submission of specimens.
Ability to work independently with minimal need for readily available supervision and communicates effectively with the veteran, families, caregivers, professionals, and other supportive staff. Provides and documents SBAR to appropriate RN and transfer of patient care as needed.
Take appropriate actions based upon communications, observations, veteran care reports, provider/staff discussions. Reports changes to provider, demonstrates the knowledge and ability to effectively intervene and recommend and implement strategies that assure optimal, safe, and effective care.
Contributes to the development of the plan of care for assigned veterans outlining program modalities and updates per policy and as changes occur. This plan of care is completed with general review and co-signature by the RN or MD, utilizing a broad knowledge base and expertise in order to develop and modify treatment plans to meet the veteran and the family’s expectations.
Familiar with how work activities impact VA’s goal to implement sound stewardship practices that are protective of the air, water, land, and other natural and cultural resources. Shall be a good steward of the environment by preventing pollution, minimizing waste, and continually improving environmental programs.

LPN - Primary Care
Works within Primary Care to provide direct care to the patient. Works collaboratively with designated providers, nursing staff to provide initial intake, clinical reminders and observe and report reason for the visit to the provider and/or RN in the clinic.
LPN – Intermittent Float
The primary role of the House Float Pool Licensed Practical Nurse intermittent (LPN) is to provide back-up coverage for the nursing staff in Primary Care (PC) including the Community Based Outpatient Clinics (CBOCs), Specialty & Acute Care (SAC) and the Surgical Clinics. The duty station is the Columbus VA but may occasionally travel to the CBOC's to provide coverage. The Part-time LPN works during the week and occasional weekends/holidays.
Duties include but are not limited to:
Communicate and interact appropriately and courteously with internal and external customers.
Maintain confidentiality of written, electronic, and verbal patient/employee information.
Work and function within a safe manner including infection control and use of equipment.
Assure that patient safety is paramount in all aspects of patient care.
Integrate and apply pertinent aspects of the Outpatient Clinic performance improvement program into his/her professional practice.
Utilize nursing research and literature in practice.
Integrate and apply aspects of the Joint Commission on Accreditation of Healthcare Organizations National Patient Safety Goals as applicable to their area of practice.
If Applicable: Schedule appointments for patients in collaboration with team members to ensure that all patients are scheduled within time constraints established by ACA guidelines and VHA policies especially adhering to priority access for service connected and OEF/OIF veteran populations
