Sabbatical for my Father - 3/23 - current

My elderly started falling and needed help with his ADL’s at the age of 90 and need to be cared for so we moved him in with us as long as possible. He is now in assisted living near us, freeing me to go back to work to help pay for my older son's medical school. He starts to UTSA Long’s SOM in July.

Diabetes Service Line, Department of Veterans Affairs - 3/19 - 3/23

One MD and I managed a panel of 33,400 diabetic veterans plus all the endocrine patients in the VISN. She primarily focused on the endocrine patients and I primarily focused on the patients with diabetes. The primary care docs sent us the sickest ones to manage, so the patients had high acuity and complexity. Many had been exposed to agent orange and were being treated as type 2 when in fact had had type 1B. We also diagnosed and treated unmanageable hypertension and hyperlipidemia. I worked with the Pharmacy and therapeutics committee to identify and manage the medications, injectables and devices need for our panel.

Assistant Professor of Medicine - Baylor College of Medicine - 6/2014 – 6/2015

Clinical accomplishments include:

Faculty appointment to the Department of Internal Medicine, Division of Endocrinology and Metabolism - responsible for the evaluation and management of diabetes patients in the Baylor College of Medicine outpatient clinic

This clinic is the tertiary referral center for the outlying St. Lukes facilities in Harris County were we see the sickest patients and the hospital follow ups for St. Lukes Hospital. I managed diabetic patients for Internal Medicine, Family Medicine, Infectious Disease, Oncology, Pulmonary, Cardiology, Nephrology, Rheumatology, Hepatology and Gastroenterology

Examples of common referrals were: sp MI with stent or CABG, sp liver, heart or kidney transplant, cystic fibrosis related diabetes, ESRD on dialysis, steroid induced diabetes, etc

Co morbidities managed include hyperlipidemia, hypothyroidism, primary and secondary hypertension, obesity, diabetic neuropathy (peripheral and autonomic), anemia, pregnancy in both gestational and pre-existing diabetics, NASH, diabetic foot ulcers, gastroparesis, etc

Established transition clinic for adolescents with type 1 diabetes who graduated from pediatrics

Perform history and physical exam, record findings, establish diagnosis and formulate a plan of care. Perform procedures as necessary.

Disetronic Medical Systems - (10/1997-10/2001)

Responsibilities included the training and management of insulin pump patients at the request of the prescribing physician. Obtained patient history and lab data to develop a dosing plan and submitted it to the physician for approval. Instructed the patient in the correct use of the insulin pump and corrected any deficient self-care skills. Calculated and instructed patients on insulin to carb ratios, sensitivity factors and how to interpret and respond to blood sugar patterns. Using carb intake, insulin dose and blood sugar data, I assisted the physician with dose adjustment and solving blood sugar problems.

Texas Lion’s Camp – (7/1998-8/2002)

On site 24-hour medical supervision of 12 boys, age 14-15 with Type 1 diabetes for 2 weeks over 5 summers. Worked one on one with each camper to assess skill levels and identify blood sugar patterns. The primary goal was to teach them how to handle their diabetes as they transition from parental supervision to independence. Areas of responsibility included: provide emergency care for severe hypoglycemia and hyperglycemia, make dose changes, maintain medical records, develop and implement age appropriate instruction focused on: tools and options for managing diabetes, intense exercise/sports, social issues, pathophysiology, diet, sick-days and self-care.

Baxter Travenol Laboratories - (2/1988-10/1997)

Responsibilities included educating hospital clinical staff in the use of IV solutions, tubing and IV access devices, parenteral nutrition, IV admixture and infusion pumps. Identify and implement product standardization and cost saving measures. Implemented partial or complete conversions of hospital IV systems and helped the clinical staff adapt their procedures to the new systems.
