A) Health insurance worth € 30,000 covering the entire stay in Poland; OR

Signed and stamped letter from the Polish company regarding future employment. The letter shall contain name, surname and the position of the signatory. 8 Certificates and documents confirming vocational education and qualifications. 9

Social Security Document (SGK) proving social security history (SGK “hizmet dökümü”) and Social Security Registration Certificate (SGK işe giriş bildirgesi”) from last workplace (with barcode or signed and stamped).

10 Civil Status Register excerpt.

11 Criminal Record Certificate.

12 Military Service Certificate.

In case of non-Turkish applicants:

Residence permit (ikamet) valid until the date of departure. 14 Copies of biometric page of passport and Polish or Schengen visas issued in last 3 years. 15 Visa fee (non-refundable even if the application is rejected). ATTENTION:

The requested documents should be up-to-date (max. 3 months old). The original documents and their copies must be presented by the applicant. Additional documents may be requested by the Consul. Application may be lodged:

- directly in the Consulate after registering in the e-konsulat system: www.e-konsulat.gov.pl,

- in visa application VFS Global centres (phone: 021*-***-**-**, www.vfsglobal.com/poland/turkey). I am aware that a failure to submit the required documents listed above, may result in a refusal of my application.
