WORK EXPERIENCES

Second Hopeful Baptist Church - Treasurer) – January 2013–Present – Maintained the financial documents for the church which included the Bank statements, Contribution Report; Individual Contribution for each member; Funds reports, and yearly or monthly statement report for each member. Each member filled out a contribution envelope with the amount that was placed inside the envelope and to which auxiliary. Counted monies and checks written for each Meeting (Sundays, Revivals, Anniversaries) day so that this information was manually entered into the Churchkeeper/Expensekeeper database/software which was purchased by the church to keep track of funds. Made/discussed on a quarterly basis all reports at meetings to the church members as a group. Provided each member with a copy of their individual contribution report.

Sutherland Global (Work from Home) – Credentialing Specialist-Fulltime) -11/30/2021-7/4/2023- Recredentialed healthcare professionals in various states for Anthem-Blue Cross Blue Shield at which they hold current hospital staff privileges. Updated each provider’s CAQH database file as according to Sutherland Anthem-Blue Cross/Blue Shield process. Reviewed the provider’s application that was submitted in CAQH to make sure of not red flags and no gaps in work experience. Maintained a grid of all providers to ensure all information was processed. This grid was provided to me by the Manager. Made sure my workload was managed effectively to meet set deadline. Interacted with other Sutherland Credentialers. Followed all rules and regulations, policies and procedures provided to me by the Sutherland Managers. Made sure the medical providers’ information was current and not expired. Primary Source verified all licenses, and certifications using that states website. Uploaded the current documents into the computers database (Smartcred system). Made sure the provider’s DEA certification was current. Reviewed the National Practitioner Databank (NPDB) for any active malpractices. After all the information was collected/uploaded to the computers system for recredentialing, the file was then received by the Manager for further processing to the Credentialing Team.

University Hospital – Medical Staff (Credentialing) Specialist (PRN) – 3/26/2019 – 11/5/2021

Currently responsible for the credentialing and privileging of Allied Health Professionals (AHPs) and Physicians. Primary source verifies all licenses, certifications, work experiences, alleged malpractices, and peer references for new AHPs and physicians requesting clinical privileges and appointment at University Hospital and University Hospital-McDuffie. The verifications are done by using State license board websites and certification board websites. This primary source verification information is entered into the MDstaff software in the providers credentials file. The providers submit the application packet by entering and uploading documents/required into the MDstaff credentialing software. I review the application and documents then transfer the providers’ information into MD staff providers’ credentials file for verification. After all information has been primary source verified, the packet is put together for review by the credentials committee and medical executive committee for approval.

Charlie Norwood VA Medical Center – Quality Management/Medical Staff Office

5/5/2013 – 3/26/2019 – Program Specialist/Medical Staff Assistant, FULL-TIME, GS-303-11

Duties: Currently maintain VetPro files on approximately 570 appointed Licensed Independent Practitioners (LIPs) for this medical center in the Medical Staff Office (MSO), Quality Management. Responsible for the recredentialing and renewal of clinical privileges and scopes of practice for all LIPs credentialed and privileged through the MSO. These providers include Physicians, Dentists, Audiology & Speech Pathologists, Physician Assistants, Nurse Practitioners, Clinical Pharmacy Specialists, Psychologists, Clinical Nurse Specialists, Psychiatrists, and Social Workers. Primary Source verification of all licenses/certification, education and training is done in an efficient manner. This information is then scanned and quality controlled (QC’d) into the VetPro database. Communicate with Human Resources (HR) staff in regards to enter on duty (EOD) dates for new hires. Make sure that HR does not hire providers without credentialing and privileging having been processed and verified through the VetPro database. Maintain credential and privileging files in the

MSO following guidelines provided by VHA Handbook 1100.19 Credentialing and Privileging (C&P) and the Medical Staff Bylaws. Assist with the Focused Professional Practice Evaluation (FPPE) and Ongoing Professional Practice Evaluation (OPPE) for Licensed Independent Practitioners (LIP) with clinical privileges or scopes of practice. These evaluations are followed in accordance with Medical Center Policy Memorandum (MCPM) #02-12-16 Professional Practice Review. Reminders to the service chief is sent on a weekly basis in regards to the healthcare provider that require an FPPE initial evaluation within a 90-day period and a OPPE evaluation within a 6 month period. Coordinates and processes the credentialing and privileging for appointment, re-appointment, and change in status of appointment for LIPs within the Veterans Health Administration (VHA) who claim licensure, registration or certification as applicable to the position. Prepared credentialing and privileging packets for new applicants. The packet included the following forms: Malpractice Form status, release of information form, declaration of health form, pharmacy signature card, and privileges required for that particular providers’ specialty. These packets were mailed to the applicant home address provided or by e-mail address. Enroll/assign LIPs access to the VetPro system in accordance with security level requirements and the Privacy Act System of Records Notice. Insures each provider enters and submits credentialing information into VetPro database and provides

assistance necessary. Make sure providers credentials (DEA licenses, state licenses, certifications and registrations, and appointments) are entered into the electronic Prescription Control Substance

database (E-PCS), PrivPlus, and VISTA systems. Responsible for the overall coordination, implementation and completion of primary source verification and data entry into the VetPro Credentialing Database. Primary Source verifications are verified through the original source of the specific credential (state license boards, certification and registration websites, and medical

facilities/outside sources). Prioritized and worked under stressful situations and demands of the Medical Staff Office to accomplish results. Assists the providers with registration for the National Practitioner Identifier (NPI), and Provider Enrollment, Chain, and Ownership System (PECOS). Demonstrated leadership required to assist the Medical Staff in the effective completion of responsibilities. Compiled data to complete Extraview reports for upper management. This data collection include information to show improvement in exit reviews completed within 7 days of provider departure from the facility. Performs query requirements to the National Practitioner Data Bank (NPDB) and the Federation of State Medical Boards (FSMB). Make sure all providers are enrolled in the Proactive Disclosure Service (PDS) and primary source verification of any adverse actions reported through queries. Enter and submit in the NPDB databank, reports on providers with alleged malpractices and/or adverse actions. Made sure the service chiefs reported adverse actions or malpractices to the Executive Committee of the Medical Staff (ECMS) for Credentialing and Privileging board members. Responsible for sending out to the service chief on a yearly basis, the individual service lines privileges template for each specialty for review and update. Made sure to run a report from PrivPlus of providers whose licenses and certifications will expire within 45 days. Also run the report within 4 months of the privileges and scopes of practice expiring. Obtains primary source verification for all licensure, certifications and registrations for all providers at time of re-appointment, change in status, and at time of licensure certification or registration renewal. Attends (ECMS) meeting for Credentialing and Privileging bi-weekly to make sure service chiefs are aware of providers needing recredentialing and reprivileging, new appointments, and evaluations. Record and type/compose ECMS minutes for review and approval of the ECMS members. These members included service chiefs, physicians/providers and the medical center Chief of Staff. Maintained the confidentiality of the ECMS minutes. Make sure all procedures/processes regarding credentialing and privileging are in accordance with the Medical Center Bylaws and Rules, and VHA Handbook 1100.19 Credentialing and Privileging.

Charlie Norwood VA Medical Center – Human Resources (HR)

9/6/2008 – 5/5/2013 - Program Specialist, FULL-TIME, GS-303-09

Duties: Managed the overall coordination of the VetPro Credentialing program in Human Resources (HR) in regards to making sure applicants are enrolled and credentialed prior to appointment to the medical center. Served as the primary VetPro point of contact and advisor for the Dependent Providers as well as Human Resource Management. Performed the administration, maintenance and operation of the credentialing process through the use of the VetPro

Credentialing Program for Allied Health Professionals (e.g. RN's, LPN's, Psychologist, Social Workers, Pharmacy Techs, Medical Technologists, Diagnostic Radiological Technologists,

Dietitians, Physical Therapists, Occupational Therapists, Respiratory Therapists). Created 2-part files for these licensed personnel at the VA Medical Center. Discussed with the incoming licensed/certified personnel on how to access the VETPRO website and which icons to access to apply for their particular position. Primary source verified with other facilities and companies the education, licenses, job position held by the individual. This verification was done either by telephone, fax or written letter. Works closely with the clinical executive service lines, and Human Resource Specialists to ensure credentialing is completed and processed before providers enter on duty at the medical center. Made sure the appointment screen in VetPro database was completed with date of appointment and status of appointment after the service chief approves the credentials, and the Board Actions was completed. Made sure Board Actions for Special Advancement for Performance, and promotions was entered into the VetPro database for the provider. Attention to details and knowledge of the medical education process is a must to ensure that no information is overlooked. This includes comparison of information on the application against what the applicant entered in VetPro and the resume’. Highlights and primary source verification of documentation to include licenses, education, certifications, and citizenship. Maintain

credentials and licenses of the Dependent providers in VETPRO system by updating the license/certification before expiration date. Made sure complete and concise documentation was done on licenses, and certifications/registrations with adverse actions. Perseverance is imperative

since rarely is required information received when first requested. Served as the primary resource in the area of credentialing for the specified occupations. Assists those in the specified occupations who request to see the information that is contained in their credentialing folder. Counseled providers on procedures and requirements of the VetPro database. The demand at times require

me to work on several different projects/professional credentials at the same time while keeping completion deadlines or assuming additional assignments. Does constant monitoring to ensure licensure, certifications are renewed at the appropriate time. Also monitored/maintained the

maintenance of VetPro files to ensure provider files were inactivated or transferred to other VA facilities and/or appointed appropriately. Independently performs work, and contacts employees,

various state licensing boards, private sector hospital staff, and other State and Federal agencies to obtain information on employees for whom credentialing records or information are required. Completes assignments independently without reminders from supervisor. Keeps supervisor informed of progress and problems. Charlie Norwood VAMC, Specialty Care Service Line

May 2008 – September 2008 = Program Support Assistant (Acting Transplant Coordinator)-FULL-TIME, GS-06 – Responsible for preparation and processing organs (heart, kidney, bone marrow, and lung) transplant referral packets. This was done by written orders or verbal orders from physicians. In this position I had to maintain lab orders, procedures (colonoscopies, EKGs, echocardiograms, etc.) on all transplant referral patients. Discussed the process of the transplant

procedure with the patient. This process included discussing labs, special procedures, and appointments to be done during the transplant referral process. Gave patients educational

pamphlets about the transplant program. Coordinated with staff in the medical center the procedures to be done for the patient per VA Central Office. These packets ranged from

approximately 15-20 packets to work with on a daily basis. Set up travel for patients that were listed on transplant list to be seen at the Transplant Centers that VA Central Office requested for where the patient to be sent to. Made sure all concerns for the patients were met and discussed with the physician as well as with the transplant centers. The packets had to be submitted to VA

Central office, Washington, D.C. within 3 months before date of labs expired; special procedures had to be no more than 9 months old. Corresponded closely on a daily basis with physicians,

nurses, techs about transplant packets. This was not only done within the organization but with outside facilities to include selected transplant centers within the United States and VA Central

Office, Washington, D.C. Knowledge of VA Organ Transplant Directive and followed guidelines as stated for transplantation of patients.

Augusta VAMC, Medical Service/Specialty Care Service Line

October 1998 – September 2008 = Secretary, FULL-TIME, GS-06 – As the Clinical Service Line Secretary I maintained the Service Line Executive’s calendar daily and made sure he was aware of events occurring on his calendar. Specialty Care consisted of approximately 70 employees, 80 Residents/Interns, and Fellows and the following Departments: Allergy, Cardiology, Rheumatology, Gastroenterology, Dermatology, Residency Training Program, Nephrology, Neurology, and Endocrinology Departments. Kept Department Secretaries informed of any changes within the service line on a need to know basis. Made sure typed material was accurately submitted in correct format as set forth in Secretary’s handbook and Director’s office. Posted on a daily basis in computer using the VISTA application the Fee-Basis employees timecards and PRN employees as well as fulltime and part-time employees’ timecards. These timecards included Title 5 and Title 38 employees. Made sure timecards were submitted accurately and on time before deadline time. Also posting of monthly organization schedule to the VA Medical Center staff was transmitted before end of the month. This organization schedule informed the medical center of on-call color

Team of the Attending and the Resident/Intern/Fellow during business hours and for after hours. Communicated on a daily basis with individuals from various backgrounds and education levels to include veterans and/or their representative, Pharmaceutical Representatives, Students, Interns/Residents, Secretaries from the Medical College of Georgia (MCG) and the VA Medical

staff. This job required me to be able to type using personal computers as well as be able to use the most up to date technology available in any office setting. Made sure the credentialing and privileging and reprivileging of physicians was completed. This included making sure the physicians completed the privileges that they were required to maintain at the medical center before

expiration of the privileges. These privileges were maintained by the medical staff office and sent to the service line for completion.

September 2001-2002 Unit Clerk, Intermittent – Admitted patients and placed patient in bed using the computer system. Thinned patient files when/if needed. Discharged patients to home or transferred to other units within the hospital. Transcribed physician orders from patient medical records. These orders included medications, consults, lab orders, transfers, discharges, admissions, etc. Sent consults to preferred specialty as ordered. Picked up blood for transfusion. Answered telephone and greet visitors to the unit. Answered patient call intercom. Maintained patient documents and confidentiality. Ordered supplies for the unit as needed to accomplish job results. Made sure Pyxis had all supplies needed for physicians and nursing personnel to accomplish the job results.

Augusta VA Medical Center, Medical Service

July 1995 – October 1998 = Secretary, FULL-TIME, GS-05 – Maintained the organization schedule for the medical center by entering the names of Residents/Interns on call for the day and night calls. Completed on a monthly basis the timesheets on each Resident/Intern and Fellow that rotated at the medical center. Hours worked by the residents had to be entered and calculated on this timesheet in order to reimburse Medical College of Georgia for Residents/Interns and Fellows rotating at the medical center.

Augusta VA Medical Center, Ward Administration

January 1992 – July 1995 = Medical Clerk, FULL-TIME, GS-04 – admitted patients and placed patient in bed using the VISTA computer system. Maintained patient files on Ward/Unit. Thinned patient files when/if needed. Discharged and transferred patients to outside nursing homes, personal care home or to private home using the VISTA computer system. Transcribed physician orders from patients medical records. These orders were of medication, consults, lab orders,

transfers, discharges, admissions, etc. Started nursing home packets for patients being transferred to nursing homes.
