I am a committed worker with excellent communication skills demonstrated by over ten years of proven success in the Human services as a care giver, Support Coordinator and community support professional and administrator in different agencies in Lancaster County and Harrisburg, Pennsylvania. And, I am willing to learn and expand my knowledge career wise and any other aspect.

I worked as a Home and Community Support Professional (CSP) - Keystone Autism Services. This is a clinically oriented position that provides support in the home and community to participants living with autism, intellectual and physical disabilities, and other behavioral issues. In my work I have learned to support my participants and not just their behaviors. I implement objectives consistent with Individual Support Plan (ISP) and Behavior Support Plans (BSP), assist in skill development, evaluation, and monitoring goal progress. I liaise with the physicians, pharmacists, social workers, families and all the stakeholders for my clients to make sure that their records and treatments are up to date.

My essential functions are.

● I do home visits where I assess and evaluate the participant’s needs based on their physical, mental, and behavioral health needs and writing up reports which I submitted to Behavior Specialist and the team so that we could come up with skill building plans for the participant. I utilized diagnostic system manual to come up with Behavior support plans for the participants which were simplified to fit the participant’s capability through task analysis.

●I write up skill building plans for clients and team members and use Functional Behavior Assessment to document and progress and attend and participate in Individual Support Plan meetings.

●I implement strategies and interventions outlined in the participant’s Behavior Supports Plans and training staff.

●I assist in coordinating, scheduling, and documenting available services and appointments as required.

●I work collaboratively with the individual’s families, power of attorney or other outside supports to ensure that the state requirements are met.

●I maintain a system of progress report of the data collected routinely.

●Read and understand the company policies as stated in the manual and adhere to them.

SKILLS

●I am a rule follower and ensure I follow policies and procedures, guidelines, standards, laws, and contractual obligations always. I maintain compliance with the policies and HIPPA regulations.

●I am detailed oriented and good in time management by completing timesheets, travel vouchers, and other required documents within a designated time.

●Competent in PCA skills grooming, scanning, taking vitals, EKG etc.
