Many years of experience as a caring registered nurse with a passion for continuous learning and

sharing my expertise with others. I have worked in a diversity of health care settings (community health,

acute care, rehab and long term care) and also have worked as an Allied Health Instructor at the Westfield Technical Academy, mentoring high school students interested in pursuing a career in healthcare.

Assume primary responsibility for a patient/family caseload that included the assessing, planning, implementing and evaluating phases of the nursing process. I initiate communication with the attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient/family regarding end of life care and appropriate support at time of death.
