Therapist/Case Manager
• Educate patients and families as to the role of Case Management and Transdisciplinary Team
• Conduct physical/psychological assessment of patients; assess goals and financial status
• Serve as back-up nurse liaison and patient advocate; ensure patient care satisfaction
• Interact daily with external case managers and physicians as to patient’s medical status, treatment plan
& home care
• Assess field evaluations on all potential patients; monitor appropriateness of service and admission criteria
• Determine adequate and timely documentation of physicians’ notes and orders
