Therapist/Case Manager

• Educate patients and families as to the role of Case Management and Transdisciplinary Team

• Conduct physical/psychological assessment of patients; assess goals and financial status

• Serve as back-up nurse liaison and patient advocate; ensure patient care satisfaction

• Interact daily with external case managers and physicians as to patient’s medical status, treatment plan

& home care

• Assess field evaluations on all potential patients; monitor appropriateness of service and admission criteria

• Determine adequate and timely documentation of physicians’ notes and orders

• Monitor on-going costs of patient care to ensure cost effectiveness of treatment plan; update status of Medicare/Medicaid and insurance benefits; perform utilization reviews

• Coordinate all aspects of discharge i.e. home health, out-patient, equipment, other facility placement, community resources, etc.

• Ensure smooth transition from facility during discharge and that proper resources are provided

• Manage financial aspects of patient care; promote payer/referral satisfaction

• Frequently attend seminars, workshops, conferences, etc.

• Prepare monthly monitoring reports for medical staff; compile data and have medical records available to physicians

• Maintain all documentation pertaining to medical staff quality assurance activities

• Coordinate and schedule third party payor billing, gathering information from all ancillary services involved in the care of patient

• Attend weekly Transdisciplinary meetings, take notes, compile information and compose correspondence resulting from each meeting

• Coordinate patient admission and hospitalization; complete all admission paperwork

• Verify benefits and secure insurance pre-certification number; ensure adequate signature on all re- certification forms

• Monitor and review status of patients Medicaid/Medicare and insurance on a weekly basis

• Function as Utilization Review Coordinator; serve as liaison between patient, family, physician and hospital staff

• Interact daily with business office as to all Medicare and insurance benefits, pre-certification, and re- certification information

• Maintain referral/inquiry/admissions log and current data on RATS systems

• Provide weekly reporting on all planned discharge and ALOS performance to Administrator

• Educate patients and families as to the role of Case Management and Transdisciplinary Team

• Supervise and administer respiratory therapy treatments according to procedures and policies

• Hands-on knowledge of sterilization and aseptic procedures as well as preparing, delivering and troubleshooting therapy equipment and supplies, medical gasses, inhaled medicine i.e. Vasodial, Bronchodial, Ventil

• Act quickly and administer proper therapy in an emergency situation

• Administer intermittent positive pressure breathing treatment, oral and tracheal suction and medications prescribed

• Check and record patient progress; keep record and prepare reports

• Familiar with the use and application of ventilators; assist with bronchoscopes, drawing & analyzing arterial blood gases

• Airway management: changing tracheotomy tubes, emergency and elective intubations
