PROFILE SUMMARY

A Registered Community Health Nurse with about 28 months’ work experience. I am highly committed to delivering quality care to patients, building a great rapport between myself and patients relatives, and making patients more informed and comfortable during care. I am highly compassionate and motivated during work with the ability to provide nurturing care to patients in a hospice setting, to ensure that they are as comfortable as possible.

I spent 384 hours averagely on the ward and in the community each academic year for three years for clinical experience, and worked in both tertiary and secondary hospitals. Main Duties

• Undertake assessment, planning and implementation of interventions for addressing patient care problems.

• Evaluate patient responses to interventions and modify plans as needed.

Bed Making

Elimination including catheterization, bowel and stoma care. Feeding e.g. oral, nasogastric or parenteral feeding and ensuring adequate nutritional intake for patients in liaison with the dietician.
