Prescribing Controlled Medication Policy

In an ongoing effort to provide safe and therapeutically appropriate medications for the people we support, Monarch has developed and implemented a Prescribing Controlled Medication Policy. The Class of Controlled Medications that are highly regulated includes:

• Benzodiazepines

• Psycho-stimulants

Benzodiazepines

Short Acting:

Alprazolam (Xanax)

Midazolam (Versed)

Oxazepam (Serax)

Triazolam (Halcion)

Psycho-Stimulants*

Mid Acting:

Lorazepam (Ativan)

Estazolam (ProSom)

Temazepam (Restoril)

Long Acting:

Chlordiazepoxide (Librium)

Clonazepam (Klonopin)

Clorazepate (Tranxene)

Diazepam (Valium)

Flurazepam (Dalmane)

Dexmethylphenidate (Focalin)

Dextroamphetamine (Dexedrine, ProCentra Liquid)

Dextroamphetamine/amphetamine (Adderall)

Methamphetamine (Desoxyn)

Methylphenidate (Concerta, Daytrana Transdermal Patch, Metadate, Methylin, Ritalin) Lisdexamfetamine (Vyvanse)

*Additional psychological testing may be required prior to/during treatment for these medications. How This May Affect You:

There is no guarantee that any controlled substances will be continued or prescribed. All medications are prescribed or continued based on the medical discretion of the provider in determining what is in the best interest of each individual person. At times, treatment may be time limited for safety reasons. Each person may be asked to provide a sample of urine for a Urine Drug Screen. This screening may be required prior to and periodically during treatment. The Urine Drug Screen may test for, but is not limited to the following substances:

• Opioids (Pain Pills)

• Amphetamines (Psycho-stimulants)

• Marijuana

• Benzodiazepines (Anti-Anxiety)

• Cocaine

Each person may be subject to review based on the North Carolina Controlled Substance Reporting System.

Each person may be subject to pill counts – please bring your medication bottles to your appointment with you. It is your responsibility to store your medications safely and these medications will not be routinely filled early.
