Assess the status of the patient's health problems and monitoring of high-risk patient medication therapy, and acting as a resource for the provision of medication information for patients, physicians, nurses, and other allied health professionals.

Advising patients about medicines, including how to take them, what reactions may occur and answering patients' questions. Follow the patient's progress to determine the effects of the patient's medications on his or her health.
