Receive patient into treatment area. Weigh patient, take temperature and blood pressure pre and post dialysis.
Document on all dialysis flow sheets. Perform and record patient observation: vital signs and all other parameters of ESRD patients, obtain blood samples and culture specimens, and prepares for shipment.
Provided health check up to designated patients under direct supervision of RN.
Gathered and documented accurate health-related information of patients.
Provided assistance to RN on evaluating and documenting overall dialysis of patients.
Monitored dialysis treatment and documented on dialysis flow charts.

Collect, label, appropriately prepare and store lab samples according required laboratory specifications.
Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate.
Obtain necessary pre and post treatment vital signs and weight. Report unusual findings to nurse supervisor.
Assist other health care members in providing patient specific detailed education regarding adequacy measures where appropriate - Online Clearance Monitoring (OLC), -Adequacy Monitoring Program (AMP), Urea Kinetic Modeling (UKM), and regarding disease process/access.
