This also improved our budget as nursing costs

went down and it allowed my RN case managers

freedom to complete all IDT notes and be able to

assist with admissions if needed

The Butterfly Program: I developed a new program

for the organization named the Butterfly Program

It was the pediatric hospice service line that focuses on palliative care with all the hospice benefits for the pediatric community

The program focuses on a palliative approach with

supportive guidance and personalized plan of care

for each individual child

The program focused on managing the patient's

illness in the home and working closely with the

physician to manage the patient to prevent hospital readmissions

As the patient's condition changes, we educated

and guided the parents

We discussed options and disease progression

along with prognosis to assist them in making

choices about quality of life

With the program's support and guidance parents

come to terms with the disease and prognosis,

hospital visits are decreased and ultimately cease, and we were successful in assisting parents in

making end of life decisions and focusing on the

quality of the child's life

I was the creator and the lead case manager for the Butterfly Program and fostered a relationship with

the staff and physicians at our local children's
