Your claim for unemployment benefits was filed on May 11, 2023. Read this letter carefully and keep it for reference during your claim; you will be held accountable for the information. You are able to establish a claim for unemployment benefits. You earned enough wages during your base period and you meet the monetary requirements. Claim Summary

You must also meet other requirements to be eligible for benefits. For example: If you quit or were fired from your job, Unemployment Insurance Agency will request information from you and your employer about your separation. A determination will be made regarding your eligibility for benefits and this will be mailed separately.

Work Registration

To complete the Register for Work requirement you must complete the following two-step process: 1. First, create your job seeker profile on Pure Michigan Talent Connect. Visit www.mitalent.org to get started.

2. Second, meet with staff from your Local Michigan Works! Service Center staff, either virtually or in-person.

The Register for Work requirement is not met until you meet with Michigan Works! Agency Service Center staff, who will verify your job seeker profile and send their verification to the Unemployment Insurance Agency (UIA). You will receive a confirmation email when the process is completed successfully.

The two steps - creating a profile and meeting with Michigan Works! Staff - must be completed and verified at least one business day before you certify for the first time to receive weekly unemployment benefits. If you fail to meet with Michigan Works! Service Center staff, it could prevent you from receiving your unemployment benefits.

After you complete the Register for Work requirement you won't need to register again until one year after you have made your initial claim for unemployment benefits. Bi-Weekly Certification

You must certify your eligibility every two weeks to receive payment. To certify, log into your Michigan Web Account Manager (MiWAM) at www.michigan.gov/uia. Your first certification must be done anytime between May 21, 2023 and May 27, 2023 and then every two weeks thereafter. After your first certification, you must continue to certify bi-weekly. You can certify in your online account seven days a week and 24 hours a day. If you do not have a MiWAM account, you can create one online at www.michigan.gov/uia. If your claim has been denied, you should continue to certify while your protest is pending; otherwise, if the protest is resolved in your favor you cannot be paid for any weeks that you did not certify.

Bi-Weekly Certification Day and Time

You can also certify by phone by calling MARVIN (Michigan Automated Response Voice Interactive Network) at 1-866-***-****. If you certify using MARVIN, you must certify on your MARVIN scheduled certification day and time.

If certifying by phone, your filing day and time are found by using the last two digits of your Social Security Number. If you are unable to certify on your scheduled day and time, you may certify on either Thursday or Friday which are make-up days.

Searching for Work

Inform UIA of Your Work Search Activities:

You are required to actively seek work and report at least one weekly work search activity during your biweekly certification for benefits. Your benefits will not be paid until you report your work search activity for the week to UIA.

• Enter your work search activity in your Michigan Web Account Manager (MiWAM) account at www.michigan.gov/uia or

• Certify with the Michigan Automated Response Voice Interactive Network (MARVIN) by calling 1-866-***-****. After completing the automated questions, you must stay on the line to be connected with an UIA agent to provide your work search information for each week. You must provide your work search activity for each week prior to benefits being released.

To avoid a delay in your certification processing by holding for an agent, you can complete your certifications online through your MiWAM account.

If you disagree with the wages reported by your employer(s), or if the employer did not report wages for a quarter in which you worked, you must complete Form UIA 1718, Unemployed Worker's Statement of Wages and return it, along with proof of earnings (W-2, check stubs, employer print out, etc.) to the address listed on the form.

Penalties

It is against state law to intentionally make false statements or conceal material information to gain or avoid the payment of benefits. You may have to repay up to 1.5 times the amount of benefits received. Benefits will be stopped, and any remaining benefits will be lost. You may also be subject to criminal prosecution. If prosecuted, you may be required to pay court costs and fines, face jail time, perform community service or any combination of these.

Protest Rights

If you disagree with this determination you have the right to protest requesting a redetermination. Your protest must be received within 30 calendar days from the mail date of this redetermination. If your protest is filed after the deadline, you must include the reason your protest is late in your statement. You can also attach copies of Monetary Eligibility

Based on the wages reported by your employer(s) from: April 01, 2022 to March 31, 2023, it is determined that you are eligible to establish a claim for unemployment benefits. Example: If your Social Security Number is ***-**-****, the last two digits are 11. Number 11 is under Monday.

You can submit your protest online at www.michigan.gov/uia through your Michigan Web Account Manager

(MiWAM) and upload documents. If you wish to protest in writing, complete Form UIA 1733, Protest of a Determination. This form is located on the website under the Forms link. Include your name, case number and social security number or Michigan Identification Number (MIN) on documents submitted with your protest. Fax the completed Form UIA 1733 and any supporting documents to 1-517-***-**** or mail to Unemployment Insurance Agency, P.O. Box 169, Grand Rapids, MI 49501-0169. If you fax or mail your protest, it must be signed.

Your protest must be received no later than June 12, 2023 or this determination becomes final. Preferred Method of Correspondence

IN WRITING: Fax correspondence to 1-517-***-**** or mail to Unemployment Insurance Agency, P.O. Box 169, Grand Rapids, MI 49501-0169. Include the date, your name, Social Security number, and signature on all correspondence. Retain a copy for yourself. Print and keep the confirmation that your Fax was received. Additional information, including a copy of UIA 1901 Handbook for Unemployed Workers and forms can be found online at www.michigan.gov/uia. Here you can also find information on calculating your weekly benefit amount and number of weeks allowed.

Your information, including but not limited to Social Security Number and Driver’s License Number, may be verified through computer matching programs.

If there is a question regarding the identity of the claim filer, UIA may conduct fact finding by contacting the filer at the number provided on the claim. If there is no immediate response, the claimant will have 48 hours to return the call. If no response is received, a determination regarding identity will be made based on the information available.
