2013-Present-Seaside Behavioral Health Center/ Outpatient Treatment Services is a distinct and organized intensive psychiatric outpatient treatment to patients who have been discharged from an inpatient psychiatric care. Treatment services are provided for patients who exhibit disabling psychiatric/psychological symptoms or experience an acute exacerbation of a severe and persistent mental disorder. Duties included the provision of psychosocial assessment, treatment plan development, group therapy, individual as needed, prepared individual and group progress notes, referrals to community resources as needed, and discharge planning.

2011-2013 LCSW-Seaside Behavioral Health Center/Partial Hospitalization Program

Serves as the director of a primary and comprehensive sub-unit, such as children's or adult services, with a wide range of direct and indirect components. Plans, organizes, and implements the services and activities of the sub-unit and coordinates same with other sub-units of the facility.

Discriminate between symptoms of emotional/behavioral disorders of organic, developmental, and psychoactive substance abuse etiology. Requires independent judgment based upon broad knowledge of social work practice, current mental health treatment modalities designed to prevent hospitalization and the skilled coordination/application of these interventions. Plans, prioritizes, and manages advanced clinical evaluation, case presentations and clinic treatment services by assessing community resources, evaluating patient’s functioning, conducting intensive interviews, gathering historical, legal, educational, and/or technical data in order to determine and recommend/implement the most effective type of psychosocial intervention needed

to assure continuity of care.

Prepare and present complex assessments and recommendations at interdisciplinary staffing, develop treatment modalities through consultation with other professionals from other disciplines. Conduct varied and complex psychotherapy with complex

multi-problem cases for patients with difficult problems. Provide specialized individual, group or family therapy requiring advanced diagnostic and treatment intervention skills in complex situations involving interpersonal, intrapersonal, social, financial, legal, and/or health dimensions. Plans, develops and leads groups designed to address inter-group relationships, development and/or conflict resolutions. Monitor and document progress of patients/consumers and make service/treatment plan modifications as deemed appropriate/necessary. Coordinate external services /act as advocate for patients to ensure continuity of services. Prepare required social work reports such as assessments, psychosocial evaluations, social histories, progress notes (response to treatment/modalities utilized), and discharge plans/summaries. Submit records to court/provide expert testimony.

ADDITONAL DUTIES AS DAY TREATMENT PROGRAM COORDINATOR: Coordinate work flow and caseload assignment of assistant coordinator. Provide direct clinical supervision of assistant coordinator. Plan/implement fund raisers for the acquisition of funds for field trips. Complete fund raiser reports/acts as treasures. Budget funds. Drive van for field trips. Evaluate program effectiveness, propose procedural changes and, with supervisory approval, implement proposed changes in order to assure proper compliance with applicable standards. Convene/chair the Interdisciplinary Staffing. Monitor response to medications, consult with the treating physician.

The LCSW has a higher degree of independence in sensitive and traumatic situations.

Perform regular case maintenance activities including documenting actions, making narrative entries, preparing necessary reports and/or case summaries, etc. Provide child welfare services to a caseload of children removed from their homes by court order or voluntary surrender and placed in foster care or a more restrictive setting. Locates/selects the most appropriate placement or

placement change. Assess service or treatment needs; continuously assess service needs of the child via interviews or observations of child; family; or, foster family. (Differs from Social Service Specialist II in the type of supervision received, independence of action, regular assignment of more complex cases, and functional supervision provided to SSSI & SSSII)

Program Office, Gary W. Project Office and the U.S. District Court. Attend Federal, Juvenile, Criminal, Municipal, and Civil Court hearings, give depositions, and submit plans to the court to enable the class members to remain in the community with rehabilitative services monitored by the case manager. Coordinate, formulate, and chair the interdisciplinary team for all Gary W. staffings. Participates in the assessment, planning, and development of Gary W. plans. Coordinate and monitor neglect/abuse complaints; and, monitor implementation of

corrective actions. Advocate on behalf of the class member in the community and via

interagency coordination to ensure protection of class members. Prepare the class member for integration into the natural system of care.
