Provide PRN coverage of shifts at a retail pharmacy located in a clinical setting. Primarily provide pharmacy services to seniors of the geriatric age group. Pharmacy services in this facility are individualized, and customer service that is prompt, courteous and efficient is highly emphasized. This Humana facility offers a range of services including: a medical clinic, laboratory services, immunization services, and a retail pharmacy unit. Pharmacists and prescribers interact all day to provide comprehensive healthcare delivery to the patients.

Kindred Hospital, Northland, Missouri (LTACH) via SOLIANT Agency

Designation: Staff Pharmacist, in-patient

Duration: June 2021 to September 2021

Review medication administration records (MARS) and profiles for patients with new orders, identify dosing issues, contact prescribers to resolve for appropriateness. Verify orders in RxConnect and perform order entry in Pro-Touch. Process orders/double check and send doses for all orders.

Compound medications, repackage medications, fill medication orders, prepare IVs and TPNs, dispense medications, and deliver items to medication rooms in nursing units or to patient medication drawers. Run inventory reports to identify low stock, restock controls and non-controls in Automatic Dispense System (MedDispense). Also run cart fill update report, fill patient medication drawers in cassettes and perform cart exchange to patient rooms on designated days.

Dose/monitor antibiotics, including aminoglycosides and vancomycin per protocol and prescriber consult. Perform pharmacokinetic calculations, renal dose adjustment, electrolyte adjustment for TPNs as needed. Order labs, follow-up reports, and do clinical documentation.

Review emails for admissions/referral information. Perform medication profile review for: prior to arrival meds (PTA), H&P, MAR, discharge medication list for prospective patients. Identify and resolve formulary, pricing, and availability issues. Liaise with admitting office and referring facility to ensure no medication issues prior to patient’s arrival to Kindred.

Assist manager with inventory management and control as follows: receive orders, review orders/invoices, unpack orders, file invoices and document inventory. Update carts and send pending doses. Assist in all aspects of inpatient pharmacy operations: check technician’s work, help train newer staff, write reports, go on codes, refill crash carts, check medication rooms refrigerators for proper storage, pull expired meds. Also take calls overnight during on-call days.

Cottonwood Springs Behavioral Health Hospital, Olathe, KS

Review prescriber orders for appropriateness: check for drug interactions, adverse drug reactions, inappropriate dosage, duplicate dosing, ensure formulary drugs are selected. Thoroughly review for accuracy/omissions, clarify orders with prescribers and document interventions in “HCS”.

Verify/confirm orders in a timely manner to enable pyxis access by nurses, and timely medication delivery to patients. Procure out of stock (OS) medications quickly from nearby health facilities or retail pharmacies to prevent delay in drug administration to patients.

Order/review labs for patients on specific medications such as Clozaril, Procrit, certain antibiotics.

Check work of pharmacy technicians, verify and sign off on unit dose repackaging, cart fills, ADS restocking etc. Perform safety checks, furnish E-kits to nursing units and replenish E-kits after use.

Perform inventory management and control. Order/receive medications (including control drugs) from vendors, reconcile invoices. Verify narcotic count, perform perpetual inventory, resolve discrepancies and document appropriately.
