Present

Function as Guardian individuals designated by the Health and Human Service Commission (HHSC), the Probate Court of various Counties within HHSC Region 9, or other entities. Duties require the application of advanced judgment to ensure that the clients from HHSC and El Paso County are taken care of. Well-timed and accurate documentation is essential in recording the services provided to the client. Work requires tolerance and sensitivity and may result in stress to moderate to considerable degrees. Health and welfare are in the client's best interest. Furthermore, the Guardian Case Manager advocates for the client, ensuring that food, shelter, and medical needs are met and that the Court's decision or requirements are met.

Well-knowledge of the Texas Estate Code, Sec. 1002.065; regular consultations with the Probate Attorney and staff established guardianship cases to testify in hearings and Probate court proceedings.

Monitors the living conditions of assigned wards/clients to ensure that he/she is receiving care, comfort, and supervision and that all of their needs are met, whether the ward resides alone, in a group home, a nursing home, or a more restrictive facility.

Conducts home and/or quality assurance visits and attends doctor appointments, meetings, and any other visits deemed necessary for the well-being of the ward/client.

Secure and inventory the ward's property, timely file at the Probate Court, and coordinate the sale, abandonment/donation of property as specified in the procedural manual and contract specifications.

Conducts complete investigation for guardianship assessments, files the application at El Paso County, prepares all documentation necessary to present to the County Probate Court, and testifies at the Probate Court hearings so that the Court makes an informed decision regarding the guardianship of an individual.

Demonstrate ability to complete written reports and make oral presentations to small or large groups.

Acquired the clients all the entitled benefits with Veterans Administration, Social Security Benefits, and Medicare, Medicaid, other HHS programs.

Caseworker Project Bravo El Paso Community Action June 3, 2009 to February 11, 2011

Perform financial screening for intake assessments to determine the client's eligibility for assistance for TDHCA-funded programs and the Community Services Block Grant CSBG.

Identify individuals of families that will be assisted with the necessary supportive services to become self-sufficient.
