COORDINATOR/ REGISTER NURSE 06/2005 to 04/2016 Hyland Nursing Home, 601 East G Street, Iron Mountain, MI 49801 Assessed residents in a skilled care facility and I completed various assessments required by the state and federal government such as: Braden scales, Oral exams, Pain Assessments, Fall risk assessments, Side-rail/restraint assessments, Bowel and Bladder assessments, Foley catheter assessments, social determination assessments, and more. Most assessments were computer based and I had to input the data into our software. Some assessments were done on paper by hand.

Completed the nursing portion of the Minimum Data Set assessment that is required by the state and federal government to obtain reimbursement for care provided to residents.

● I initiated the nursing portion of care plans and updated care plans for residents. Led and participated in resident care conferences with residents and their families/guardians/spouses/insurance managers/significant others along with the interdisciplinary team (Dietary Manager, MDS Nurse/Nursing, Diversional Therapy, Physical Therapy, Occupational Therapy, Speech Therapy, Social Worker, and Physician).

Fountain Blvd., Colorado Springs, CO 80910 I was a Registered Nurse who was part of an interdisciplinary team of Psychiatrists, Psychologists, Social workers, School Teachers, Mental Health Techs, and other nurses. I took care of teens and children with behavioral disorders, psychiatric disorders, truancy, and other gang or drug related problems. I provided direct care through individual and group counseling. I implemented specific medical or psychiatric treatment procedures and behavioral therapy program approaches. I was responsible for writing, evaluating, and revising nursing care treatment pans for individual patients. I was responsible for keeping the environment safe and keeping the milieu calm. I also administered prescribed medications, led some activities on the unit, provided 1:1 communication and teaching as needed. In emergency situations, I needed to care for resident's medical needs, as well as their safety needs. In violent crisis situations, I had to assess the need for seclusion, need for restraints, or other consequences for bad choices or behaviors in order to keep individuals safe. Our goal for these patients were to stabilize them and to get them back into the community with the proper support; instead of ending up in prison, ending up dead ending up on the streets, or ending up in gangs. Some teens and children were in the facility related to depression, other psychiatric diagnoses, or behavioral issues/acting out negatively due to family problems like divorce. We taught the teens/children many coping skills to deal with stress, anger, depression, and other feelings.

I worked as a full time charge nurse RN2/Acting RN3 on weekend rotation. As a RN charge nurse I assessed, planned, implemented, and evaluated nursing care for patients on a whole ward. In crisis situations, when a resident was a harm to themselves or others, I had to assess the need for 1:1, seclusion, and last resort restraints. I would have to notify the psychiatrist and psychologist on duty in crisis situations. I had to document the situation, the interventions used, the results, and outcome.
